?Para cuales condiciones o enfermedades se prescribe este medicamento?

La cetirizina se usa para aliviar temporalmente los sintomas de la fiebre del heno (alergia al polen, el polvo y otras sustancias suspendidas en el aire) y las alergias a otras sustancias (como acaros del polvo, caspa de animales, cucarachas y mohos). Entre los sintomas se encuentran: estornudos; escurrimiento nasal; comezon, enrojecimiento o lagrimeo de ojos; y comezon en la nariz o la garganta. La cetirizina se usa tambien para tratar la picazon y el enrojecimiento de la urticaria. Sin embargo, la cetirizina no previene la urticaria ni otras reacciones alergicas de la piel. La cetirizina pertenece a una clase de medicamentos llamados antihistaminicos. Actua bloqueando la accion de la histamina, una sustancia presente en el organismo que provoca los sintomas de alergia.

La cetirizina tambien se vende en combinacion con pseudoefedrina (Sudafed, otros). La informacion de esta monografia se refiere unicamente al uso de la cetirizina sola. ?Como se debe usar este medicamento?

La cetirizina viene en forma de tabletas normales, tabletas masticables y jarabe (liquido) que se toman por via oral. Suele tomarse una vez al dia, con o sin alimentos. Tome la cetirizina mas o menos a la misma hora todos los dias. Siga cuidadosamente las instrucciones de la receta y pidales a su medico o farmaceutico que le expliquen cualquier cosa que no entienda. Tome la cetirizina tal como se lo indiquen. No aumente ni disminuya la dosis, ni la tome con mas frecuencia que la indicada en la etiqueta del envase o la recomendada por su medico.

No use cetirizina para tratar una urticaria con hematomas (moretones) o ampollas, de color extrano o que no da comezon. Si tiene estos tipos de urticaria, llame a su medico.

Deje de tomar la cetirizina y llame a su medico si su urticaria no mejora durante los 3 primeros dias de tratamiento o si dura mas de 6 semanas. Si desconoce la causa de su urticaria, llame a su medico.

Si esta tomando cetirizina para tratar la urticaria y aparece cualquiera de estos sintomas, solicite de inmediato asistencia medica de emergencia: dificultad para tragar, hablar o respirar; hinchazon en la boca y alrededor de ella o hinchazon de la lengua; sibilancia (jadeo); babeo; mareos, o perdida del conocimiento. Estos pueden ser sintomas de una reaccion alergica llamada anafilaxia, que pone la vida en riesgo. Si el medico sospecha que puede tener anafilaxia con la urticaria, quizas le recete un inyector de epinefrina (EpiPen). No use la cetirizina en lugar del inyector de epinefrina.

Seventh Pay Commission seeks one-month extension from finance ministry

New Delhi: The Seventh Pay Commission, headed by justice A. K. Mathur, has sought a one-month extension from the finance ministry and is preparing to submit its report by the end of September. The commission is unlikely to recommend the lowering of the retirement age as rumoured earlier or push for lateral entry and performance-based pay.

The commission, set up once in every 10 years to review pay, allowances and other benefits for central government employees, was appointed by the previous government on 28 February 2014 and was asked to submit its report in 18 months, which falls on 31 August.

“There are some data points that are missing, which we hope to get by this month end. We are trying to submit the report by 20 September,” an official of the commission said, speaking on condition of anonymity.

The Sixth Pay Commission had submitted its report a little ahead of its deadline on 24 March 2008. The revised pay scales were implemented retrospectively starting 1 January 2006, while recommendations relating to allowances were implemented prospectively.

The finance ministry apprehends that salary and pension expenditure will both rise by around 16% in 2016-17 as a result of the implementation of the Pay Commission recommendations. This may allow capital expenditure to grow by no more than 8% during the year, leaving little room to aggressively push for an infrastructure build-up.

“The Pay Commission impact may have to be absorbed in 2016-17. The phase of consolidation, extended by one year, will also be spanning out in this period. Thus, in the medium-term framework, the fiscal position will continue to be stressed,” the finance ministry said in the 2015-16 budget presented in February.

The official cited earlier said the Pay Commission report needs to be effective from 1 January 2016, or by April 2016 at the latest.

“It will be the government’s prerogative when to implement it. But beyond 1 January 2016, there will be arrears. But then, the government will be subject to criticism. Earlier, they had hidden behind Pay Commissions giving late reports,” he added.

However, the official said the commission is likely to maintain the status quo on the retirement age of central government employees, currently 60 years. “We are not going to either recommend lowering or raising the retirement age. If we lower the age limit, the pension burden will bust the government’s medium-term fiscal targets,” he added.

Asked whether government has sent any directives to the commission on the kind of hike it can afford, the official said the message it has got broadly is to keep the hikes low. “Merge the basic with dearness allowance, don’t stretch it beyond—that is the message. But that is a good message for the government to send. But there is no pressure otherwise. In fact, there is a lot of cooperation,” he said.

The official said merging basic pay with dearness allowance, which is mandatory, would itself mean a 155% rise for central government employees. “We have to decide how much to give above that. So, it will look good if you compare basic to basic,” he added.

On whether the commission will recommend performance-based pay bands, he said it will make some feasible recommendations, though he couldn’t guess if the government would accept them. The Sixth Pay Commission had also recommended performance-based pay revisions, but the government is yet to implement them.

“Eighty-eight percent of central government employees are industrial and non-industrial workers working with railways, post, paramilitary and army. So, performance-based pay revision is the wrong instrument for them. Biggest growth in government services is in paramilitary forces, where staffs in Central Reserve Police Force and Central Industrial Security Force have gone up by 75-80% in the last 10 years. By the time we have dealt with them, the bureaucracy is an afterthought. It does not affect anything,” he added.

D. K. Joshi, chief economist at rating agency Crisil Ltd, said the government is expected to be restrained in its pay hikes this time around, given the low inflation level and tepid growth momentum. “The last two Pay Commissions had significantly bumped up demand and fiscal deficit. But the government is unlikely to be populist this time. It has already showed restraint in the hike in minimum support prices for farmers,” he said.

However, Joshi said the Pay Commission will have a permanent income effect as well as a one-time impact through the payment of arrears, which will lead to increase in demand for consumer durables.

Docusate is a stool softener. It makes bowel movements softer and easier to pass.

Docusate is used to treat constipation, and to reduce pain or rectal damage caused by hard stools or by straining during bowel movements.

What is the most important information I should know about Sulfalax Calcium (docusate)?

Before using docusate, tell your doctor if you have an intestinal disorder such as Crohn's disease or ulcerative colitis, or stomach pain, nausea, or vomiting.

Do not take mineral oil or aspirin while using docusate, unless otherwise directed by your health care provider. Call your doctor if your symptoms do not improve or if they get worse after using this medicine for 1 to 3 days.

What should I discuss with my healthcare provider before using Sulfalax Calcium (docusate)?

You should not use this medication if you are allergic to docusate.

If you have certain conditions, you may need a dose adjustment or special tests to safely use this medication. Before using docusate, tell your doctor if you have:

an intestinal disorder such as Crohn's disease or ulcerative colitis; or

stomach pain, nausea, or vomiting.

This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment. It is not known whether docusate passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I use Sulfalax Calcium (docusate)?

Use this medication exactly as prescribed by your doctor. Do not use it in larger amounts or for longer than recommended. Follow the directions on your prescription label.

Take this medication with a full glass of water. Do not crush, chew, break, or open a docusate capsule. Swallow the pill whole.

Measure liquid medicine with a special dose-measuring spoon or cup, not a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one. Mix the liquid with 6 to 8 ounces of milk, fruit juice, or infant formula and drink the mixture right away.

Wash your hands before and after using docusate rectal enema.

Try to empty your bowel and bladder just before using the enema.

Use only the applicator provided with the medication to insert it into your rectum.

For best results from the enema, lie down on your left side for at least 30 minutes after using the enema to allow the liquid to distribute throughout your intestines. Avoid using the bathroom during this time.

Call your doctor if your symptoms do not improve or if they get worse after using this medicine for 1 to 3 days. Store docusate at room temperature away from moisture and heat.

What happens if I miss a dose?

Take the missed dose as soon as you remember. If it is almost time for your next dose, wait until then to take the medicine and skip the missed dose. Do not take extra medicine to make up the missed dose.

What happens if I overdose?

Seek emergency medical attention if you think you have used too much of this medicine.

Overdose symptoms may include nausea, vomiting or stomach pain.

What should I avoid while using Sulfalax Calcium (docusate)?

Avoid using laxatives or other stool softeners unless your doctor has told you to.

Zusatzliche empfangnisverhutende Ma?nahmen sollten in den ersten 14 Tagen nach dem ersten Zyklus eingenommen werden oder wenn eine Tablette versehentlich verpasst. Schmierblutungen konnen auftreten, aber in der Regel spontan stoppen. Der Patient wird empfohlen, ihren Arzt zu konsultieren, wenn langerer und anhaltende Blutungen auftreten. Laktation. Uberempfindlichkeit gegen einen der Inhaltsstoffe. Bekannte oder vermutete Brustkrebs. Patienten mit Lebererkrankungen oder bosartig oder verschwenden Krankheiten. Patienten mit klassischen Migrane sollten nicht mit einem kombinierten oralen Kontrazeptivum. Relative Kontraindikationen (diese Patienten wurden arztlicher Aufsicht erforderlich): Diabetes mellitus, Tetanie, Bluthochdruck, Epilepsie, Chorea, Multiple Sklerose, Nierenfunktionsstorungen, Asthma, Depressionen, Krampfadern, kardiale Dysfunktion, Erkrankungen der Gallenblase, Patienten, die Kontaktlinsen und adiposen Patienten oder andere Bedingungen durch Flussigkeitsretention beeinflusst tragen. Die Risiko-Nutzen sollte bei Patienten mit Endometriose und Myome in Betracht gezogen werden. Es wird darauf hingewiesen, dass die Verwendung von Ginette-35 abzusetzen, wenn eine der folgenden Symptome auftreten:

Der Beginn der Migrane oder starke Kopfschmerzen in einem neuen Patienten.

Wenn es einen allmahlichen oder plotzlichen Verlust des Seh-oder Horvermogen.

Bei den ersten Anzeichen von thromboembolischen Erkrankungen, zB Lungenembolie, zerebrovaskularer Insuffizienz, Thrombophlebitis, Hirnblutung, retinale Thrombosen, koronarer oder mesenterialen Thrombose.

Take this medication by mouth. as directed by your doctor, with a full glass of water (8 ounces / 240 milliliters). If stomach upset occurs, take with food or milk. Older adults may be more sensitive to the side effects of this drug, especially skin reactions, blood disorders, easy bleeding/bruising, and a high potassium blood level.

Patients with AIDS may be more sensitive to the side effects of this drug, especially skin reactions, fever, and blood disorders.

This drug passes into breast milk. While there have been no reports of harm to healthy infants, this drug may have undesirable effects on infants who are ill or premature or have certain disorders (jaundice, high blood levels of bilirubin, G6PD deficiency). This medication may make you more sensitive to the sun. Avoid prolonged sun exposure, tanning booths or sunlamps. Use a sunscreen and wear protective clothing when outdoors.

If you have diabetes, this product may affect your blood sugar levels. Check your blood sugar levels regularly as directed by your doctor. Tell your doctor right away if you have symptoms of low blood sugar (see Side Effects section). Your doctor may need to adjust your diabetes medication, exercise program, or diet.

Older adults may be more sensitive to the side effects of this drug, especially skin reactions, blood disorders, easy bleeding/bruising, and a high potassium blood level.

Patients with AIDS may be more sensitive to the side effects of this drug, especially skin reactions, fever, and blood disorders.

This medication is not recommended for use during pregnancy especially near the expected delivery date because of possible harm to the unborn baby. Consult your doctor for more details.

This drug passes into breast milk. While there have been no reports of harm to healthy infants, this drug may have undesirable effects on infants who are ill or premature or have certain disorders (jaundice, high blood levels of bilirubin, G6PD deficiency). Therefore, breast-feeding is not recommended for infants with these conditions. Consult your doctor before breast-feeding.

Interactions

Drug interactions may change how your medications work or increase your risk for serious side effects. This document does not contain all possible drug interactions. Keep a list of all the products you use (including prescription/nonprescription drugs and herbal products) and share it with your doctor and pharmacist. Do not start, stop, or change the dosage of any medicines without your doctor's approval.

Some products that may interact with this drug include: "blood thinners" (such as warfarin), dofetilide, hydantoins (such as phenytoin), live vaccines, methenamine, methotrexate.

Although most antibiotics are unlikely to affect hormonal birth control such as pills, patch, or ring, a few antibiotics (such as rifampin, rifabutin) can decrease their effectiveness. This could result in pregnancy. If you use hormonal birth control, ask your doctor or pharmacist for more details.

This product may interfere with certain laboratory tests, possibly causing false test results. Make sure laboratory personnel and all your doctors know you use this product.

Overdose

If overdose is suspected, contact a poison control center or emergency room right away. US residents can call their local poison control center at 1-800-222-1222. Canada residents can call a provincial poison control center. Symptoms of overdose may include: severe nausea/vomiting/diarrhea, severe dizziness or drowsiness, mental/mood changes.

If taking this medication for an extended period, laboratory and/or medical tests (such as complete blood count, kidney function tests, potassium blood level, cultures) should be performed periodically to monitor your progress or check for side effects. Consult your doctor for more details.

Missed Dose

If you miss a dose, take it as soon as you remember. If it is near the time of the next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.

Storage

Store at room temperature away from light and moisture. Do not store in the bathroom. Keep all medications away from children and pets.

Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed. Consult your pharmacist or local waste disposal company.

They affect 4% to 6% of the population, i. e. 3 million French people and 25 to 30 million Europeans. 80% of these diseases are of genetic origin. For many years, the French Muscular Dystrophy Association (AFM-Téléthon) has been involved in research into rare diseases, and particularly in the development of innovative drugs. The Association also fights to ensure that rare diseases remain a public health priority.

Of the trials supported by AFM-Téléthon, nearly half of them involve non-neuromuscular rare diseases. Each of these trials is strategic because it will stimulate progress in developing treatments for other diseases. With the first successes already achieved.

It was more than a fondness but he had never admitted the truth out loud and the truth was: Sollux Captor was truly madly deeply flushed for Eridan Ampora. Never once had Eridan asked him to fill a quadrent and the mustard blood didn't know whether to be relieved or upset about this. Considering he has asked just about every other troll to do so he was completly crushed. Eridan considered them close enough to be morials but seeing as Sollux already had one, this wasn't official. The Gemini knew he had to hide his feelings somehow and the easiest way to do this was to pretend he hated the troll. He had to pretend too often for his liking and it crushed him everytime he did so. Oh how he hated to pretend but did he have a choice? He certainly didn't think so. With shaky fingers he typed back:

How to use Vital-D Rx

Product Description Common use Amaryl is an oral blood sugar-lowering drug which belongs to the class of sulfonylureas. Active substance is Glimepiride. It is used together with diet and exercise to treat type 2 (non-insulin dependent) diabetes. The mechanism of its action is to stimulate the secretion and release of insulin from beta cells of the pancreas (pancreatic effect). Amaryl is used to treat non-insulin depending diabetes (type 2) as monotherapy or in combination with insulin (or metformin).

Dosage and direction

The best dose is determined by a physician on a basis of regular measurements of blood sugar content.

Be careful your sugar does not fall too low due to stress, skipping a meal, exercising too long, or drinking alcohol. Know signs of hypoglycemia and keep a source of sugar at hand. If your sugar level is too high you may feel very thirsty or hungry. You may also urinate more than usual. Take this medication exactly as prescribed as it is a part of a complicated program consisting of diet, medication, and exercise routines. Tell your doctor if you are pregnant or plan to become pregnant, have adrenal or pituitary gland problems before taking this medication.

Contraindication Amaryl is contraindicated in patients with history of diabetic ketoacidosis, diabetic coma and prekoma, insulin-dependent diabetes (type 1), severe renal dysfunction, severe hepatic impairment, individual hypersensitivity to Amaryl, other members of sulfonylurea class and sulfonamides.

Possible side effect If you have any signs of allergic reaction to this medication such as hives, swelling of face and throat, rash or other major symptoms (severe skin rash, itching, redness, pale skin, easy bruising or bleeding, numbness, dark urine, clay-colored stools, abdominal pain, fever, nausea)seek for immediate medical help.

Drug interaction Diuretics like hydrochlorothiazide (Hydrodiuril, Ezide, Hydro-Par, Microzide, furosemide (Lasix)), corticosteroids such as prednisone and methylprednisolone (Medrol), phenytoin (Dilantin), niacin, and sympathomimetics such as pseudoephedrine (Sudafed) are able to increase blood sugar and diminish effect of Amaryl. Propranolol (Inderal) and atenolol (Tenormin) belong to beta-blockers and also are able to affect blood levels and decrease activity of Amaryl.

Missed dose Take the missed dose as soon as you remember about it. If it is almost time of your next dose just skip it and return to your regular schedule.

Overdose Immediate medical attention is needed in case you took too much medicine. Possible side effects include: extreme weakness, confusion, blurred vision, stomach pain, trouble speaking, tremors, sweating, seizure, and coma.

Storage Store at room temperature in a tight container and keep away from sunlight, moisture, kids and pets.

Benefits and Uses of White Oak Bark

The white oak bark is a hardwood native to eastern North America. It comes from the Quercus Alba tree. Although its name refers to it as “white,” it actually has a light gray color. It can last approximately 200 to 300 years, with some older specimens such as the famous Wye Oak in Wye Mills, Maryland which is estimated to be over 450 years old.

For centuries, the hardwood has been used to treat a variety of health conditions. It was listed in the United States Pharmacopoeia for its astringent and antiseptic properties. It has also been approved as a diarrhea treatment by the German Commission E – a government agency that oversees the use of herbal products.

It is known as Common Oak or Chene Commun, Chene Anglais, Chene Blanc or Chene Blanc d’Amerique, and Chene Pedoncule. In some areas, it is called the Durmast Oak, the Corteza de Roble, or the Ecorce de Chene.

What is terbinafine?

Terbinafine is an allylamine medicine used to treat fungal infections. It is especially effective against dermatophytes (tinea infections).

The original brand is Lamisil®. In New Zealand (2016) it is available as 250 mg tablets (Dr Reddy's Terbinafine, fully funded by PHARMAC) and Terbinafine-DRLA. The tablets require a doctor's prescription. Topical options include creams, gel, solution and spray from various manufacturers, and are available at a pharmacy without prescription.

How does terbinafine control fungal infections?

Terbinafine inhibits a fungal enzyme, squalene epoxidase, and stops the cells making ergosterol, the main component of the cell wall.

What is the usual dose regime for terbinafine?

Terbinafine cream is applied to the affected area twice daily for one to four weeks.

The oral dose of terbinafine for adults is 250 mg daily.

For children, the tablets can be hidden in food – the tablets taste unpleasant:

Weight 10-20 kg, 62.5 mg per day

Weight 20-40 kg, 125 mg per day

Weight >40 kg, 250 mg per day

Sometimes, if the fungal infection does not clear, the dose in children may need to be increased.

Treatment can be repeated if necessary.

What is the metabolism of terbinafine?

Terbinafine is absorbed well when taken orally, with or without food. It is bound to proteins such as albumin in the circulating blood and becomes concentrated in fat cells and within skin and nails. It is slowly eliminated in faeces and urine mostly after conversion by the liver into inactive compounds. It is a moderate CYP2D6 inhibitor. Doses may need to be reduced in the presence of kidney disease.

Skin concentrations may be up to 75-fold higher than those in the blood. It may persist in the skin for up to 8 weeks after the drug has been discontinued and in the nails for up to a year.

Side effects of terbinafine

Terbinafine appears to be a relatively safe drug. Side effects, usually minor, arise occasionally. Serious side effects occur rarely.

Adverse reactions to terbinafine may include:

Nausea, feeling of fullness.

Diarrhoea, abdominal pain.

Headache.

Dizziness.

Painful muscles and joints

Taste disturbance, loss of appetite (uncommon, <1%).

Liver disease (rare, <0.01%).

Reduced neutrophil white blood cell count (very rare, <0.01%).

Allergic skin rash including urticaria (common, <10%) and Stevens-Johnson and toxic epidermal necrolysis (very rare, but potentially dangerous, <0.01%).

Terbinafine should not be taken in pregnancy or during breast-feeding.

Drug interactions with terbinafine

Terbinafine does not generally alter the concentration of other medications.

Rifampicin results in a slight decrease in the concentration of terbinafine. Cimetidine may slightly increase the concentration of terbinafine. Terbinafine may increase the concentration of aripiprazole, brexpiprazole, darifenacin, eliglustat, vortioxetine, tricyclics and several other medications. It may reduce the analgesic effectiveness of codeine and similar drugs. Terbinafine is not thought to interact with the oral contraceptive pill .

For the local treatment of oropharyngeal candidiasis and vaginal yeast infections, also used in fungal infections of the skin such as ringworm, athlete's foot, and jock itch.

Canifug Mechanism Of Action:

Canifug interacts with yeast 14-α demethylase, a cytochrome P-450 enzyme that converts lanosterol to ergosterol, an essential component of the membrane. In this way, clotrimazole inhibits ergosterol synthesis, resulting in increased cellular permeability. Canifug may also inhibit endogenous respiration, interact with membrane phospholipids, inhibit the transformation of yeasts to mycelial forms and the uptake of purine, impair triglyceride and/or phospholipid biosynthesis, and inhibit the movement of calcium and potassium ions across the cell membrane by blocking the ion transport pathway known as the Gardos channel.

What Bisoprolol tablets are and what they are used for

Bisoprolol fumarate belongs to a group of medicines called beta-blockers. It is used to treat: • high blood pressure • angina pectoris (pain in the chest caused by blockages in the arteries leading to the heart).

What you need to know before you take

Do not take Bisoprolol tablets if you

are allergic to bisoprolol tablets or any of the other ingredients of this medicine (listed in section 6) suffer from severe asthma or from other severe breathing difficulties have acute heart failure or are in shock caused by heart problems suffer with heart conduction or rhythm problems (2nd or 3rd degree AV-block, sick sinus syndrome or sinoatrial block) have a slow heart rate of less than 60 beats per minute before starting treatment. have low blood pressure suffer from severely blocked blood vessels, including blood ciruculation problems (which may cause your fingers and toes to tingle or turn pale or blue) suffer from increased acidity of the blood (metabolic acidosis) suffer from untreated phaeochromocytoma (high blood pressure due to a tumour of the adrenal medulla, a gland located near the kidney). are already treated with floctafenine (product for pain) or sultopride (product for mental illness). Warnings and precautions Talk to your doctor, pharmacist or nurse before taking Bisoprolol tablets: • • • •

Bisoprolol tablets may increase your hypersensitivity to the substances you are allergic to and increase the severity of allergic reactions.

If you suffer from a tight, painful feeling in the chest in periods of rest (Prinzmetal’s angina). Bisoprolol tablets may increase the number and the length of attacks suffer from treated phaeochromocytoma (high blood pressure due to a tumour of the adrenal medulla, a gland located near a kidney) have or have suffered from psoriasis (severe skin rashes) suffer from blood circulation problems (in the fingers, toes, arms and legs), including less severe forms of Raynaud's phenomenon and intermittent claudication (a cramp like pain in the calves brought on by exercise or walking). The complaints may be worse, particularly at the beginning of the treatment suffer from a certain heart conduction disorder (so-called 1st degree AV block).

If you are going to have an anaestethic, please tell your doctor or dentist that you are taking Bisoprolol tablets. It may be necessary to stop taking this medicine up to 48 hours before receiving the anaesthetic.

Other medicines and Bisoprolol TabletsOther medicines may be affected by bisoprolol fumarate. They in turn may affect how well bisoprolol fumarate works. Bisoprolol can interact with: • medicines used for the treatment of pain and which exhibit an anti-inflammatory or fever inhibiting effect (NSAIDS) such as floctafenine. • medicines used for the treatment of psychiatric disorders (anxiety, psychoses or depression) such as sultopride, MAO-A inhibitors, tricyclic antidepressants, phenothiazines (also used for vomiting and nausea) and barbiturates (also used for epilepsy). • medicines used for controlling the blood pressure or medicines used for heart problems such as calcium antagonists, centrally acting anti-hypertensives (e. g. clonidine, methyldopa, guanfacin, moxonidine, rilmenidine), anti arrhythmics (e. g. disopyramide, quinidine, amiodarone), digitalis glycosides, sympathicommimetics (e. g. isoprenaline, dobutamine, noradrenaline, adrenaline) other beta-blockers (including eye drops which are used for the treatment of increased eye pressure). • medicines with a stimulating action on a certain part of the nervous system (parasympathicomimetics) which, amongst others, are used for the treatment of Alzheimer’s disease (e. g. tacrine). • medicines used for anaesthesia during surgery. • medicines used to treat migraine (e. g ergotamine). • a certain medicines that muscle relaxant (baclofen). • a medicines that decreases the side effects of cancer treatment (amifostine). • a certain medicines for malaria prevention (mefloquine). • adrenal cortex hormones that have, amongst others, an anti-inflammatory action (corticosteroids). • iodated contrast products, used for making certain organs and blood vessels visible on a scan. • sympathomimetics that activate both beta - and alpha-adrenoceptors (e. g. norepinephrine, epinephrine) The combination of bisoprolol fumarate and any of the drugs listed above, may influence the blood pressure and/or heart function. • medicines used for the treatment of diabetes; for example, insulin and anti-diabetic medicines in tablet form. Bisoprolol fumarate may increase the blood sugar lowering effect and can mask the symptoms of low blood sugar content. Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.

Pregnancy and breast-feeding Bisoprolol tablets are not recommended during pregnancy or breastfeeding. If you are pregnant or breast-feeding, think you may be pregnant or planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.

Driving and using machines This medicines could have a minor influence on the ability to drive and use machines. Bisoprolol tablets may make you feel tired and dizzy. Make sure you are not affected before you drive or operate machinery, particularly at start of treatment, after any change in dose or medicine and if taken with alcohol.

Bisoprolol tablets contains lactose If you have been told that you have an intolerance to some sugars, contact your doctor before taking this medicine, as it contains lactose. 2

Method of administration: The tablet should be swallowed with a sufficient amount of fluid (e. g. one glass of water) at the same time each day. The tablet can be taken with or without food. Always take this medicine exactly as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure. The recommended doses are: • Adults: Initial dose: Your doctor will start the treatment with the lowest possible dose. Sometimes 5 mg per day (24 hours) can be sufficient. 10mg once daily with a maximum recommended dose of 20mg. • Patients with kidney disease: patients with severe kidney disease should not take more than 10mg once daily. This dosage may eventually be halved and divided into two 5mg doses • Patients with liver disease: patients with severe kidney disease should not take more than 10mg once daily. This dosage may eventually be halved and divided into two 5mg doses • Elderly: it is recommended to start with the lowest possible dose. • Children under 12 years old and adolescents: Bisoprolol tablets are not recommended for use in children below 12 years old due to a lack of data.

If you take more Bisoprolol tablets than you should If you have accidentally taken more than the prescribed dose, contact your nearest casualty department or tell your doctor or pharmacist at once. Symptoms of overdose are: slowed heart beat, asthma, low blood sugar, reduced blood pressure (possibly causing you to feel faint, dizzy or light-headed) and acute heart failure (fluid retention, breathlessness and tiredness).

If you forget to take Bisoprolol tablets If you forget to take a dose, take it as soon as you remember, then go on as before. Do not take a double dose to make up for a forgotten dose.

If you stop taking Bisoprolol tablets Do not suddenly stop taking Bisoprolol tablets as this could cause your condition to worsen (an exacerbation of a disorder of the heart may occur or your blood pressure may become high again). Your doctor will reduce the dose gradually or replace it by another medicine. If you have any further questions on the use of this medicine, ask your doctor, pharmacist or nurse.

Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them. Stop treatment and contact a doctor at once if you have symptoms of an allergic reaction such as itchy skin rash, flushing, swelling of the face, lips, tongue or throat, or difficulty breathing or swallowing. This is a very serious but rare side effect (occurs in less than 1 in 1000 patients). You may need urgent medical attention or hospitalisation. Tell your doctor if you notice any of the following side effects or notice any other effects not listed: • Common (occurs in less than 1 in 10 users but more than 1 in 100): tiredness, exhaustion, dizziness and headache (especially at the beginning of treatment). These are generally mild and often disappear within 1-2 weeks. Feeling of coldness or numbness in the extremities (Raynaud’s disease), increase of existing limping (intermittent claudication), low blood pressure, feeling or being sick, diarrhoea, abdominal pain, constipation. • Uncommon (occurs in less than 1 in 100 users but more than 1 in 1000): dizziness or fainting when standing up due to low blood pressure, lack of muscle strength, sleep disturbances, depression, slow or irregular heart beat, worsening of heart failure, muscle weakness and cramps, joint disease. Patients with asthma or a history of breathing problems may experience difficulty in breathing. • Rare (occurs in less than 1 in 1000 users but more than 1 in 10,000): loss of consciousness, nightmares, hallucinations, hearing impairment, dry eyes (to be considered if you wear contact lenses), reduced sexual 3

performance, inflammation of the lining of the nose caused by an allergy, increased levels of fat (triglycerides) in your blood’, low blood sugar levels (hypoglycaemia), increased levels of enzymes in your blood (ALAT, ASAT) which indicate how your liver is functioning), inflammation of the liver causing yellowing of the skin or eyes (hepatitis), the appearance of certain blood cells could cause symptoms of lupus syndrome such as joint swelling, fever, tiredness, hayfever (allergic rhinitis), skin rash, (symptoms usually disappear when treatment is stopped). • Very rare (occurs in less than 1 in 10,000 users): conjunctivitis (red, sore, itching or weeping eyes), psoriasis-like rash or worsening of psoriasis, hair loss, shock due to low blood sugar levels. If you get any side effects, talk to your doctor, pharmacist or nurse. This includes any side effects not listed in this leaflet.

Keep this medicine out of the sight and reach of children. Do not take this medicine after the expiry date which is stated on the carton and blister label after EXP. The expiry date refers to the last day of that month. Do not store above 30°C. Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help to protect the environment.

What Bisprolol tablets contains -

The active substance is Bisoprolol fumarate. Each Tablet contains contains either 4.24mg or 8.49mg bisoprolol equivalent to either 5mg or 10mg Bisoprolol fumarate the other ingredients are lactose monohydrate, microcrystalline cellulose, magnesium stearate, crospovidone. The 5mg tablet contains a yellow pigment blend 22812 (which contains lactose and iron oxide yellow (E172)). The 10mg tablet contains a beige pigment blend 27215 (which contains lactose and iron oxide yellow and red (E172)).

What Bisprolol tablets look like and contents of the pack Blisters comprising of PVC/PVdC/aluminium foil, contained within a printed carton box. The 5mg tablets are mottled pale yellow, round and convex, with ‘BI’ ‘above a breakline with ‘5’ below. The 10mg tablets are mottled beige, round and convex, with ‘BI’ above a breakline with ‘10’ below. The tablets can be divided into equal halves. Pack sizes: 10, 20, 28, 30, 50, 56, 60 or 100 tablets. Bifen IT with Bifenthrin 7.9% is a flowable multi-purpose insecticide. Bifen IT has a long residual control with a product label of over 75 insects. Since Mosquitoes and Ticks like to habitat in dense foliage and shrubs, don't forget to spray the vegetation as well as the lawn. Repeat this application monthly.

Mole Cricket Control The nymphs of mole crickets cause damage in your grass during the summer months. The mole cricket nymph looks like an adult but is wingless. The nymphs are easier to kill than the adults. Begin treatment with Bifen IT or Bifen LP of the infested area towards the end of April or beginning of May in the Southern coastal states. Use Bifen towards the end of May for the Carolina coastal areas. Timing is important for mole crickets since the immature stages of the Mole Crickets do the most damage.

Flea Control Use Bifen IT for flea control outside. Mix 1 oz per 2-4 gallons of water per 1,000 square feet of outside surface area to kill adult fleas and flea larvae. If larvae is present, use a higher volume of water to penetrate the soil.

One drawback of insecticide granules, is that it doesn't work as quickly as the Bifen IT. Many customers use Bifen IT for mosquito control in their back yards. Mosquitoes feed on plant juices. You find them on the leaves of plants, bushes, and trees. Try to spray the underside of the leaves. Remember that Bifen IT is non selective and will kill the good guys such as bees and butterflies so avoid spraying any flowering parts because that is where the bees go. We need to save the bees. The 3/4 gal size is the best value, but consider the one gal container because it has a tip and measure device. The tip and measure makes life easier when mixing.

It has been over six years now and I have no bugs in my home at all. I use it every six months and it keeps all insects away from our house and yard. Great for controlling fleas and ticks in your yard as well. I only spray perimeter of house and on bushes and plants my dogs don't eat or chew like grass. Works great.

We also had some ants coming into the house somewhere but I thought that would require something else to get rid of them. I sprayed Bifen IT from a 2 gallon pump sprayer first around the perimeter of our house's foundation then gradually worked my way outward into the yard spraying the grass and the undersides of the leaves on all shrubs and trees, and two days later no mosquitoes and to my surprise no more ants. Great product for sure just be careful with it. Spray it on a clear day, no wind and wear gloves, googles and a mask to be safe.

I had a serious ant problem around a previous pluming repair inside of a bathroom wall. I touched the mound and it exploded like a volcano with ants. With product in hand I sprayed the mound. I checked the area 2 days later and no movement. I cleaned the entire area a week later and still no more ants. All repairs are complete and I will continue to treat the area to keep the pests away.

I n the past we have had a lot of problems with spiders coming inside the house during the fall and winter months. I mixed up a batch and sprayed a three foot wide strip all the way around the outside perimeter of our house and within just a couple of hours any insect that crossed that area was dead. Even after rain and snow the barrier is still intact. The bugs don't stand a chance, even the many different kinds of spiders that live in and around the hedges in my yard. Even if they make it inside, they die before going more than a foot or so inside the house. Best long term insecticide I've ever used.

Review by Jeff / (Posted on 10/10/2015) Good stuff I use this to "refresh" the termite and bug exclusion barrier around my house in central Florida, and to spray the lawn when I see evidence of chinch bugs or other pests. I use a flat shovel to make a narrow trench along the concrete slab, 10 feet at a time, mix the recommended amount in a 5 gal pail with a hose connected on the bottom, and flood the trench, then move 10 feet to do it again. For the lawn I mix the recommended amount of Bifen in a tow-behind powered sprayer for the area I'm treating, and drive the lawn tractor over the area until the tank is empty. I generally do not use it on plants otherwise because the bees and butterflies are susceptible. Also, it's not approved for use on fruits and vegetables. It seems to have some residual effect - remains effective until there's heavy rain.

Tiny, the size of the dot on an i. Big mamas full of babies. You name it they were in my house and on my dog. First application was to my yard and all baseboards inside my house. Also bathed my dog. They were still there, everywhere. We were still pulling between 50-75 per day from her. Next application was double strength and I used quite a bit. Bingo. Very few around the house. Still a few on my dog but manageable. I will do one more application this weekend and wipe out all of them, hopefully. I gave this a 4 because it has reduced the tick population drastically but it hasn't eliminated them. But using extra strength worked well.

A year ago I got a hunting camp in Southern Alabama that was so over run with oriental roaches I wouldn't sleep there. It's surrounded by pines and these roaches live in the fallen needles. After one 2' perimeter granular application, I ran out of product, and one liquid interior application the roaches are nearly gone. Anything that finds it's way into the cabin dies within minutes. I find big spiders curled up in the bathroom. Fire ants don't stand a chance against it. I use it in an old apartment building in South Florida and have had wonderful results controlling palmetto bugs, ants and never see spiders. The services I was paying to spray for spiders and other insects were pretty much ineffective and often had to call them back to redo the work more than once. I looked online and found DIYPC and the product Bifen and started spraying myself. The results were instant and I spray about every three months. It only takes about 20 minutes. Also had an infestation of caterpillars eating my oleander bushes. Sprayed those and got rid of the problem (no flowers on the plants when I sprayed so no bees were harmed). One quart will cover my entire house twice this fall. Keep animals away while wet, but once it's dry, I just sweep up the dead bugs. So far it's lasted for two weeks. I measured the number of dead bugs in a single square foot, 53. if that's the density around the rest of house, I swept up 13,000 dead stink bugs today. and I did it myself.

Since my elderly mother is allergic to wasp and bee stings (and I hate the loathsome things myself), I needed a product to kill the wasps in our mobile home's eaves by the front door (and elsewhere) so she would not get stung going in and out. I emailed DIYPC and explained how my situation had changed since the last time I ordered from them, needing to get a different product that would work but would also be gentle on our new cat. Ken emailed me back promptly, and since he uses Bifen IT at his home and also has pets, he recommended this product.

It arrived at about 10 in the morning and since I had to go to work at 2, I dropped everything and sprayed then and there. I used it with an attached hose sprayer and went to war with the wasps. The wasps were not happy, to say the least, but only one or two actually almost got to me (I was a man on a mission because you do not mess with my Momma!!). I was a little worried, because while it did have near instant knockdown, sometimes they would keep flying for a second or two. However, the next day the wasps were gone. That was about 2 weeks ago, and still no wasps. I sprayed heavier than I probably should have, but I did not want to get stung and also wanted to see all their hideouts.

As a bonus, the ants we had tried every home remedy known to man to get rid of are now gone as well. It took longer for them because they were embedded in the walls, but I soaked our exterior walls top to bottom with this stuff. The number of ants dropped drastically, but a few were still showing up. For the third day in a row now though, no one in my family has seen a single ant. Excellent!!

I don't like chemicals, but I love my mother and it just does not seem right for insects to try and take over if I can do something about it. I will be buying another bottle soon, as I used up all of this bottle on our home and a very small portion of our 5-acre yard, and of course it will be from Ken and DIYPC.

I noticed that it was only taking them 5 actual minutes to spray and before I could say, "i'd like to walk around with you, they were done. My husband just happened to stop and talk to one of the clerks at their Do It yourself store in Atlanta who sold him this product. Well, we've been on the same bottle for the past 2-3 (1 oz or so to one gallon - just follow the directions closely) years and the results have been the same as when we used the expensive company. Only better, because now i take my spray bottle and spray my patio area and around my mailbox. Believe me, once you spray an area, no more pests. You will not be sorry on this product. I love it.

Using during the spring and summer for the mosquitos as directed keeps the mosquito population under control for at least 30 days. Bifen IT does not degrade in sunlight like Malathion does. It is deadly for mosquitos. Use 0.5 oz. -1.0 oz./gallon water and DO NOT retreat for at least one week. Also in the fall ladybeetles will start to come inside the upstairs of our house. I spray the top eves when I see them begin to congregate and usually one application does it!

Maybe Cinch bugs and Mole Crickets combined. Followed directions and treated 3 times for egg/hatch cycle. Grass now looks great! These treatments also got rid of Fire Ants for a long time even though my neighbor’s lawn is invested with Fire Ants, the ants have not come back! I treated my dog owning neighbor’s lawn and cleared it of ticks. Super product, I'm buying more this year. Easy to apply with water hose and Ortho water hose sprayer.

I ordered the 4 3/4 gals. and the day I received I sprayed the entire outside of house and yard. During spraying I didn't have one Mosquito bite me and before I counldn't even go in my yard without Mosquito spray on. It has been 6 weeks and still no mosquitos or any type of ant or insect in my yard. My neighbow across the street was swarmed by flying termites one evening and he said to me I don't see them at your place and i told him about Bifen IT and actually gave him some to try and they were gone as soon as he sprayed, I was raised on a farm in the country and have never found a product as good as this. La claritromicina es un antibiotico macrolido con actividad bactericida indicado en el tratamiento de infecciones de vias respiratorias superiores e inferiores, infecciones de la piel y otitis media causadas por microorganismos sensibles.

La claritromicina se utiliza en ninos y adultos para el tratamiento de faringitis y tonsilitis causadas por Streptococcus pyogenes , tambien se utiliza en el tratamiento de sinusitis bacterial aguda causada por Haemophilus influenzae. Branhamella ( Moraxella , principalmente Neisseria ) catarrhalis o S. pneumoniae .

El medicamento esta indicado en ninos y adultos para el tratamiento de la otitis media aguda causada por Haemophilus influenzae , B. catarrhalis o S. pneumoniae ; para el tratamiento de infecciones dermicas causadas por Staphylococcus aureus o Streptococcus pyogenes asi como en infecciones diseminadas complejas causadas por Mycobacterium avium y otras micobacterias atipicas.

Tambien es util en el tratamiento de infecciones causadas por Chlamydia trachomatis .

FARMACOCINETICA Y FARMACODINAMIA EN HUMANOS :

La claritromicina inhibe la sintesis proteica en organismos susceptibles penetrando la pared celular y uniendose a las subunidades ribosomicas 50S, inhibiendo con esto la translocacion de la RNA - aminoacil transferasa y la sintesis polipeptidica.

Absorcion: La claritromicina se absorbe rapidamente en el tracto gastrointestinal. La biodisponibilidad absoluta despues de administrar una tableta de 250 mg reportada fue de aproximadamente 50 a 55%. Los alimentos causan un ligero retraso en el inicio de la absorcion de la claritromicina y en la formacion del metabolito 14- hidroxiclaritromicina ; no obstante, la cantidad total absorbida del farmaco no se ve afectada por la ingestion concomitante con los alimentos.

Al administrar una tableta de 250 mg de claritromicina a adultos sanos alimentados, el pico en la concentracion serica se obtuvo en las primeras 1 a 4 horas promediando 0.6 µg / ml. Con una administracion sostenida cada 12 horas, el estado estacionario en el pico de la concentracion de clindamicina se obtuvo en un periodo de 2 a 3 dias y promedio 1 o 2-3 µg / ml con dosis de 250 o 500 mg respectivamente. El pico en las concentraciones sericas del metabolito principal 14- hidroxiclaritromicina promedio alrededor de 0.7 µg / ml y se obtuvo aproximadamente 2 horas despues de la ingestion de 250 mg de claritromicina.

Estudios preliminares en hombres sanos sugieren que la relacion entre las areas bajo la curva de claritromicina a 14- hidroxiclaritromicina en estado estacionario es de aproximadamente 3:1.

El farmaco es ampliamente metabolizado principalmente en el higado. Aunque se han identificado al menos 7 metabolitos de la claritromicina ; el metabolito principal detectado en el suero es 14- hidroxiclaritromicina y es el unico con una substancial actividad antimicrobiana.

Distribucion: Son limitados los datos disponibles de la distribucion en humanos, al parecer tanto el farmaco como su metabolito se distribuyen en la mayoria de los tejidos y fluidos corporales. Debido a las altas concentraciones intracelulares, las concentraciones del farmaco en los tejidos son mayores que las concentraciones sericas.

Estudios in vitro a concentraciones terapeuticas normales han reportado que la union de la claritromicina a proteinas se presenta en 42 a 72%. Esta union a proteinas de la claritromicina y su metabolito 14- hidroxiclaritromicina disminuye al incrementarse la concentracion serica del farmaco.

Eliminacion: La eliminacion de la claritromicina parece seguir una farmacocinetica dosis-dependiente no lineal posiblemente como resultado de la limitada capacidad metabolica. Al administrar a adultos sanos una dosis unica de 250 mg o dosis de 1.2 g la vida media de eliminacion encontrada, promedio de 4 a 11 horas respectivamente. Durante tratamientos con dosis multiples cada 12 horas la vida media de eliminacion reportada para claritromicina se incremento de 3 a 4 horas con dosis de 250 mg a 5 o 7 horas con dosis de 500 mg ; la vida media del metabolito 14- hidroxiclaritromicina se incremento de 5 a 6 horas con dosis de 250 mg a 7 o 8.7 horas con dosis de 500 mg. La depuracion renal de la claritromicina es relativamente independiente de la dosis y se aproxima a la velocidad de filtracion glomerular normal.

La claritromicina se elimina por mecanismos renales y no renales.

Siguiendo una administracion oral de una dosis unica de 250 mg de claritromicina radiomarcada en individuos sanos, despues de 5 dias aproximadamente 38% de la dosis (18% como claritromicina ) se excreto en la orina y 40% en heces (4% como claritromicina ). La vida media serica de la claritromicina se prolonga en pacientes con dano en la funcion renal. El farmaco puede usarse sin ajustar la dosis en pacientes con dano hepatico con funcion renal normal.

El medicamento esta contraindicado en pacientes con hipersensibilidad conocida a la claritromicina. eritromicina o cualquier otro antibiotico macrolido.

Al igual que otros agentes antiinfecciosos el uso de la claritromicina puede resultar en una sobrepoblacion de bacterias u hongos no susceptibles, en estos casos debe instituirse una terapia adecuada a fin de combatir la infeccion.

Debido a que la claritromicina se excreta principalmente por el higado y el rinon, debe tenerse precaucion al administrarse a pacientes con insuficiencia renal, debiendo reducirse la dosis o prolongarse el intervalo entre las mismas.

En caso de insuficiencia hepatica aislada, no suele ser necesario ajustar la dosis.

RESTRICCIONES DE USO DURANTE EL EMBARAZO Y LA LACTANCIA :

No existen estudios adecuados y controlados en humanos. A la fecha, no se ha dilucidado claramente el riesgo potencial al feto.

Este medicamento esta contraindicado en el embarazo, especialmente en el primer trimestre de gestacion; al igual que durante el periodo de lactancia, ya que se excreta por la leche materna.

REACCIONES SECUNDARIAS Y ADVERSAS :

Los efectos secundarios comunmente reportados fueron leves y transitorios e involucraron principalmente al tracto gastrointestinal; algunos de estos efectos incluyeron diarrea, nausea, sabor anormal de los alimentos, dispepsia, malestar abdominal, vomito, glositis, estomatitis y constipacion.

Otros efectos secundarios reportados fueron: hepatomegalia. disfuncion hepatica, incremento en el tiempo de protrombina. dolor de cabeza, mareo, insomnio, urticaria y erupciones en la piel.

INTERACCIONES MEDICAMENTOSAS Y DE OTRO GENERO :

Teofilina : Se ha asociado un incremento en las concentraciones sericas de teofilina con el uso conjunto de claritromicina y teofilina probablemente como resultado de la limitada capacidad metabolica en el higado y/o a la depuracion de la teofilina.

Terfenadina : Se ha reportado en algunos pacientes que recibieron concomitantemente terfenadina y claritromicina prolongacion del intervalo QT y taquicardia ventricular incluyendo torsades des pointes.

Carbamazepina : La claritromicina puede incrementar el area bajo la curva tiempo-concentracion serica para la carbamazepina.

ALTERACIONES EN LOS RESULTADOS DE PRUEBAS DE LABORATORIO :

En algunos pacientes se han reportado elevacion en las concentraciones sericas de ALT (SGPT), AST (SGOT). - glutamiltransferasa. fosfatasa alcalina, LDH y/o concentracion total de bilirrubina. Tambien se puede incrementar el tiempo de protrombina.

PRECAUCIONES EN RELACION CON EFECTOS DE CARCINOGENESIS, MUTAGENESIS, TERATOGENESIS Y SOBRE LA FERTILIDAD :

La claritromicina no ha exhibido potencial mutagenico en diversos estudios in vitro incluyendo la prueba con microsomas de mamiferos, prueba de frecuencia de mutaciones inducidas en bacterias, ensayo de la sintesis de DNA en hepatocitos de rata, ensayo en linfoma de raton. En la prueba in vitro de aberracion cromosomica la claritromicina presento en una prueba resultados positivos debiles y negativos en otra.

Los resultados en la prueba de AMES realizados con varios metabolitos de la claritromicina tambien fueron negativos.

A la fecha no se han efectuado estudios a largo plazo para evaluar el potencial carcinogenico de la claritromicina. tampoco se conoce el efecto del farmaco sobre la fertilidad en humanos.

DOSIS Y VIA DE ADMINISTRACION :

La via de administracion de la claritromicina es oral, puede administrarse con o sin alimentos. La dosis usual es de 250 mg cada 12 horas o 500 mg cada 24 horas. En pacientes con sinusitis aguda a media, exacerbaciones de bronquitis cronicas causadas por H. influenzae o infecciones complicadas de Mycobacterium avium se recomienda administrar 500 mg cada 12 horas.

La duracion de la terapia para la mayoria de las infecciones es de 7 a 14 dias; sin embargo, esta dependera del tipo y severidad de la infeccion y debera ser establecida por el medico tratante de acuerdo a la respuesta del paciente.

Dosis en dano hepatico y renal: la claritromicina generalmente puede administrarse sin necesidad de ajustar la dosis a pacientes con dano hepatico y funcion renal normal. Sin embargo, en pacientes con depuracion de creatinina menor a 30 ml / min con o sin dano hepatico debe considerarse la reduccion en la dosis o prolongar el intervalo de tiempo entre una dosis y otra. El tratamiento sugerido para pacientes con depuracion de creatinina menor a 30 ml / min se inicia con una dosis de 500 mg seguida de 250 mg dos veces al dia, esta terapia corresponde a una dosis de 500 mg dos veces al dia para pacientes con funcion renal normal.

MANIFESTACIONES Y MANEJO DE LA SOBREDOSIFICACION O INGESTA ACCIDENTAL :

Los efectos por sobredosis se reportan principalmente en el tracto gastrointestinal. En caso de sobredosis se recomienda aplicar medidas de soporte bajo vigilancia medica. Si el paciente esta consciente se debe inducir el vomito.

La claritromicina no es removida por hemodialisis o dialisis peritoneal.

RECOMENDACIONES SOBRE ALMACENAMIENTO :

Conservese a temperatura ambiente a no mas de 30 °C y en lugar seco. Protejase de la luz.

Eltroxin 100mcg Tablets - Summary Of Product Characteristics (Spc), Eltroxin

4.1 Therapeutic indications

Recommended clinical indications: Control of hypothyroidism, congenital hypothyroidism in infants . acquired hypothyroidism in children and juvenile myxoedema.

4.2 Posology and method of administration

In younger patients, and in the absence of heart disease, a serum Levothyroxine (T4) level of 70 to 160 nanomols per litre, or a serum thyrotrophin level of less that 5 milli-units per litre should be targeted. A pre-therapy ECG is valuable because ECG changes due to hypothyroidism may be confused with ECG evidence of cardiac ischaemia. If too rapid an increase in metabolism is produced (causing diarrhoea, nervousness, rapid pulse, insomnia, tremors, and sometimes anginal pain where there is latent cardiac ischaemia,) dosage must be reduced, or withheld, for a day or two, and then re-started at a lower dose level.

Adults: Initially 100 micrograms daily, preferably taken before breakfast or the first meal of the day. Adjust at three to four week intervals by 50 micrograms until normal metabolism is steadily maintained. he final daily dose may be up to 100 to 200 micrograms.

Elderly: As for patients aged over 50 years.

For patients over 50 years, initially, it is not advisable to exceed 50 micrograms daily. In this condition, the daily dose may be increased by 50 micrograms at intervals of every 3-4 weeks, until stable thyroxine levels are attained. The final daily dose may be up to 50 to 200 micrograms.

Patients over 50 years with cardiac disease:

Where there is cardiac disease, 25 micrograms daily or 50 micrograms on alternate days is more suitable. In this conditions, the daily dose may be increased by 25 micrograms at intervals of every 4 weeks, until stable thyroxine levels are attained. The final daily dose may be up to 50 to 200 micrograms.

For patients aged over 50 years, with or without cardiac disease, clinical response is probably a more acceptable criteria of dosage rather that serum levels.

The maintenance dose is generally 100 to 150 micrograms per m 2 body surface area. The dose for children depends on their age, weight and the condition being treated. Regular monitored is required to make sure he/she gets the right dose. Infants should be given the total daily dose at least half an hour before the first meal of the day.

Congenital hypothyroidism in infants:

For neonates and infants with congenital hypothyroidism, where rapid replacement is important, the initial recommended dosage is 10 to 15 micrograms per kg BW per day for the first 3 months. Thereafter, the dose should be adjusted individually according to the clinical findings and thyroid hormone and TSH values.

Acquired hypothyroidism in children:

For children with acquired hypothyroidism, the initial recommended dosage is 12.5-50 micrograms per day. The dose should be increased gradually every 2 to 4 weeks according to the clinical findings and thyroid hormone and TSH values until the full replacement dose is reached.

Juvenile myxoedema in children:

The initial recommended dosage is 25 micrograms daily. In such conditions, the daily dose may be increased by 25 micrograms at intervals of every 2 - 4 weeks, until mild symptoms of hyperthyroidism is seen. The dose will then be reduced slightly.

Tablets are to be disintegrated in some water (10 to 15 ml) and the resultant suspension, which must be prepared freshly as required, is to be administered with some more liquid (5 to 10 ml).

• Hypersensitivity to levothyroxine sodium and any components of Eltroxin tablets

• Adrenal gland disorder or adrenal insufficiency

4.4 Special warnings and precautions for use

Levothyroxine should be introduced very gradually in patients aged over 50 years (see section 4.2) and those with long standing hypothyroidism to avoid any sudden increase in metabolic demands.

Patients with panhypopituitarism or other causes predisposing to adrenal insufficiency may react to levothyroxine treatment, and it is advisable to start corticosteroid therapy before giving levothyroxine to such patients.

Levothyroxine sodium should be used with caution in patients with cardiovascular disorders, including angina, coronary artery disease, hypertension, and in the elderly who have a greater likelihood of occult cardiac disease.

An ECG before starting treatment with levothyroxine is advised, as changes induced by hypothyroidism may be confused with evidence of ischaemia.

Thyroid replacement therapy may cause an increase in dosage requirements of insulin or other anti-diabetic therapy (such as metformin). Care is needed for patients with diabetes mellitus, and diabetes insipidus.

Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption should not take this medicine.

Subclinical hyperthyroidism may be associated with bone loss. To minimise the risk of osteoporosis, dosage of levothyroxine sodium should be titrated to the lowest possible effective level.

Parents of children receiving thyroid agent should be advised that partial loss of hair may occur during the first few months of therapy, but this effect is usually transient and subsequent regrowth usually occurs.

4.5 Interaction with other medicinal products and other forms of interaction

Interactions affecting other drugs:

Levothyroxine increases the effect of anticoagulants (Warfarin) and it may be necessary to reduce the anticoagulation dosage if excessive, hypoprothrombinaemia and bleeding are to be avoided.

Blood sugar levels are raised and dosage of anti-diabetic agents may require adjustment.

Tricyclic anti-depressants (e. g. amitriptyline, imipramine, dosulepin) response may be accelerated because levothyroxine increases sensitivity to catecholamines; concomitant use may precipitate cardiac arrhythmias.

The effects of sympathomimetic agents (e. g. adrenaline or phenylephrine) are also enhanced

If levothyroxine therapy is initiated in digitalised patients, the dose of digitalis may require adjustment. Hyperthyroid patients may need their digoxin dosage gradually increased as treatment proceeds because initially patients are relatively sensitive to digoxin.

False low plasma concentrations have been observed with concurrent anti-inflammatory treatment such as phenylbutazone or acetylsalicylic acid and levothyroxine therapy.

Beta Blockers: levothyroxine (thyroxine) accelerates metabolism of propranolol, atenolol and sotalol.

Isolated reports of marked hypertension and tachycardia have been reported with concurrent ketamine administration.

Interactions affecting Levothyroxine:

Amiodarone may inhibit the de iodination of thyroxine to tri iodothyronine resulting in a decreased concentration of tri iodothyronine, thereby reducing the effects of thyroid hormones.

Anti-convulsants, such as carbamazepine and phenytoin, enhance the metabolism of thyroid hormones and may displace them from plasma proteins.

Initiation or discontinuation of anti-convulsant therapy may alter levothyroxine dosage requirements.

Effects of Levothyroxine may be decreased by concomitant sertraline.

Absorption of levothyroxine (thyroxine) possibly reduced by antacids, proton pump inhibitors, calcium salts, cimetidine, oral iron, sucralfate, colestipol, polystyrene sulphonate resin and cholestyramine (administration should be separated by 4-5 hours).

Metabolism of levothyroxine (thyroxine) accelerated by rifampicin, barbituarates, and primidone. (may increase requirements for levothyroxine (thyroxine) in hypothyroidism)

Imatinib: plasma concentration of levothyroxine (thyroxine) possibly reduced by imatinib.

Beta blockers may decrease the peripheral conversion of levothyroxine to triiodothyronine. Oestrogen, oestrogen containing product (including hormone replacement therapy)and oral contraceptives may increase the requirement of thyroid therapy dosage. Conversely, androgens and corticosteroids may decrease serum concentrations of Levothyroxine-binding globulins.

A number of drugs may affect thyroid function tests and this should be borne in mind when monitoring a patient on levothyroxine therapy.

4.6 Pregnancy and lactation

The safety of Levothyroxine treatment during pregnancy is not known, but any possible risk of foetal abnormalities should be weighed against the risk to the foetus of untreated hypothyroidism.

Levothyroxine is excreted in breast milk in low concentrations, and it is contentious whether this can interfere with neonatal screening.

4.7 Effects on ability to drive and use machines

4.8 Undesirable effects

Side-effects are usually indicative of excessive dosage and usually disappear on reduction of dosage or withdrawal of treatment for a few days. Such effects include:

General: Headache, flushing, fever and sweating

Immune system disorders: hypersensitivity reactions including rash, pruritus, dyspnoea, joint pain, malaise and oedema

Metabolic: weight loss

Nervous system: tremor, restlessness, excitability, insomnia. Rarely, benign intracranial hypertension in children.

Cardiac: anginal pain, cardiac arrythmias, palpitations, tachycardia

Gastrontestinal: diarrhoea, vomiting

Musculoskeletal and connective tissue: muscle cramps, muscle weakness, craniostenosis in infants and premature closure of epiphysis in children.

Reproductive: menstrual irregularities

Heat intolerance, transient hair loss in children, also reported.

Some patients may experience a severe reaction to high levels of thyroid hormone. This is called a "thyroid crisis" with any of the following symptoms:

• Hyperpyrexia, tachycardia, arrhythmia, hypotension, cardiac failure, jaundice, confusion, seizure and coma

Reporting of suspected adverse reactions:

Reporting suspected adverse reactions after authorisation of the medicinal product is important. It allows continued monitoring of the benefit/risk balance of the medicinal product. Healthcare professionals are asked to report any suspected adverse reactions via the Yellow Card Scheme at: www. mhra. gov. uk/yellowcard.

In most cases there will be no features. Signs of an overdose may include: fever, chest pain (angina), racing or irregular heartbeat, muscle cramps, headache, restlessness, flushing, sweating, diarrhoea, tremor, insomnia and hyperpyrexia. These signs can take up to 5 days to appear . Atrial fibrillation may develop. Convulsions occurred in one child. There may be increased toxicity in those with pre-existing heart disease.

Give oral activated charcoal if more than 10mg has been ingested by an adult or more than 5mg by a child, within 1 hour. If more than 10mg has been ingested by an adult or more than 5mg by a child, take blood 6-12 hours after ingestion for measurement of the free thyroxine concentration. The analysis does not need to be done urgently but can wait until the first working day after the incident. Patients with normal free thyroxine concentrations do not require follow up. Those with high concentrations should have outpatient review 3-6 days after ingestion to detect delayed onset hyperthyroidism. Features of clinical hyperthyroidism should be controlled with beta-blockers, e. g. propranolol.

5. Pharmacological properties

5.1 Pharmacodynamic properties

Eltroxin is a tablet containing the hydrated form of Levothyroxine sodium which is used for the treatment of hypothyroidism. The Thyroid gland is dependant upon 2 active principles for it's main hormone activity. These are Levothyroxine (Tetraiodothyronine) and Tri-iodothyronine (see Goodman and Gilman, 1985). These closely related iodine containing amino acids are incorporated into the glycoprotein thyroglobulin. The chief action of these hormones is to increase the rate of cell metabolism. Levothyroxine is deiodinated in peripheral tissues to form Tri-iodothyronine which is thought to be active tissue form of thyroid hormone. Tri-iodothyronine is certainly more rapid acting and has shorter duration of action than Levothyroxine.

5.2 Pharmacokinetic properties

Levothyroxine sodium is incompletely and variably absorbed from the gastrointestinal tract. It is almost completely bound to plasma proteins and has a half-life in the circulation of about a week in healthy subjects, but longer during pregnancy in patients with myxoedema. A large portion of the Levothyroxine leaving the circulation is taken up by the liver. Part of a dose of Levothyroxine is metabolised to triiodothyronine. Levothyroxine is excreted in the urine as free drug, deiodinated metabolites and conjugates. Some Levothyroxine is excreted in the faeces. There is limited placental transfer of Levothyroxine.

Leeloominai Lekatariba Lamina-Tchai Ekbat De Sebat (A. K.A "Leeloo") is one of the Supreme Beings of the Universe. She was sent to Earth in corporeal form to become the Fifth Element, the final piece of a powerful weapon to destroy the Great Evil.

History Edit

In 1914, a race of mechanoid aliens arrive at an ancient Egyptian temple to collect the four elemental stones and a bronze sarcophagus containing the body of the Fifth element in human form. Knowing that the weapon was designed to defeat a dark entity known as the Great Evil every 5,000 years, the aliens, called Mondoshawans, promise the temple's current human priest that they would return the weapon's components to Earth before the Great Evil arrived three centuries later. They leave the priest with a single bronzed key, which is to be used to open the secret room in the temple for arrangement of the weapon. The priest promises to keep it safe, passing his knowledge on to his successors until they return.

Three centuries later, in a dystopian future where most of the green planet known then had become industrialized by humans, the current priest, Vito Cornelius, preserves the key in his apartment. Along with his apprentice, David, he patiently awaits the arrival of the stones and the Fifth Element. The Mondoshawans, as promised, are en route to Earth to return the stones and sarcophagus, when their ship is attacked by a fleet of Mangalores. The Mondoshawans and their ship are destroyed, leaving nothing but a mangled right forearm from the sleeping Supreme Being in the wreckage. Discovered by Earth scientists, the severed forearm is transported to a laboratory in New York and analyzed, and its DNA isolated and studied. The DNA, far superior to human DNA which contains 40 "memo groups", contains over 200,000 "memo groups." The scientist describes the DNA of the being as being "tightly packed, with infinite genetic knowledge. Almost as if this being was. engineered."

He continues to describe the cells as being "perfect."Being that a few cells were still viable in the arm, the lead scientist begins reconstructing the being using the isolated DNA and a complex machine that puts the being together part by part, beginning with the skeleton, followed by the muscles and organs, and finally exposing the body to ultraviolet radiation, forcing the body to take measures to protect itself from UV damage by growing skin and hair. The entire process is explained by the lead scientist to a skeptical General Munro, who is less than enthusiastic about the being's revival, informs the scientist that he hopes the the being is "friendly", stating his intent to destroy the being with a card-activated destruction system inside the machine if he is not. When the shield covering the being during its immersion in UV light is removed, General Munro appears awestruck to see a perfectly-formed human woman. The woman is awakened by the flash of an overhead camera, gasping and panicking while attempting to exit her confinement in the chamber of the machine. Taking note of the humans watching her, she attempts to communicate with them in a rapid, unknown language. As General Munro approaches her, she states another unknown word in a warning tone. Tapping on the window, Munro tells her that if she wishes to leave the chamber, she would need to improve her ability to communicate. Angered, the woman stares at him menacingly, growling deep in her throat, before finally putting her fist through the glass chamber wall, which had been described previously as "unbreakable." Knocking Munro out, she quickly accesses the controls outside the chamber and escapes the laboratory through the ventilation system. The dumbfounded scientist simply stared ahead, uttering only one word: "Perfect."

Police are quickly dispatched to round up the escaped woman. She is discovered to have no file or identity in their onboard systems, and make attempts to arrest her. She then dives from the ledge of the building, where she is trapped, and falls through the roof of Korben Dallas's cab. From there, Korben makes a great effort to defy police orders to turn her over to them at her pleading in broken English. They manage to escape into the thick fog at the city floor. Before passing out, Leeloo pleads with Korben to take her to a priest named Vito Cornelius. Meanwhile, the Great Evil, taking the form of a massive, sentient, dark planet, travels across the universe to reach Earth, occasionally contacting Zorg to check on his progress with obtaining the stones so his attempts to destroy Earth won't be interrupted.

Once rendezvoused with Cornelius, who immediately recognizes her as the Supreme being he is waiting for, the woman is roused from unconsciousness by Korben, who has taken it upon himself to kiss her gently. Holding a gun to his head, she repeats angrily, "Seno ekto gamat!" Cornelius later translates this to mean, "Never without my permission." It is during this scene that she introduces herself as Leeloominai Lekatariba Lamina-Tchai Ekbat De Sebat. Korben, unable to pronounce her lengthy title, asks her for a shorter name, to which she responds, "Leeloo."

Leeloo explains to Cornelius and David that the Mondoshawans had given the elemental stones to someone they trusted, and that she was meant to contact this individual on her arrival. Careful tracking reveals that this individual was located on planet Fhloston in the Angel constellation. Cornelius immediately begins making preparations to get on a ship to this location.

After tricking Korben out of his winning tickets to Fhloston Paradise on the planet Fhloston, Cornelius sets David and Leeloo up at the boarding station to board the ship as a married couple, with David posing as Korben. Korben arrives soon after, intimidating David and reclaiming his tickets. He claims that Leeloo is his wife and guest on the trip, and they board the ship that takes them to a luxurious hotel in Fhloston Paradise. Cornelius sends David ahead to the temple after discovering what happened, and stows away on the ship in its ventilation system. Leeloo, during this trip, having finally learned English, makes contact with the individual carrying the stones, Diva Plavalaguna, who tells her that she will soon give her what she came for. Some time later, Leeloo fights a troop of brutish Mangalores who have arrived to claim the stones as well, set to an appropriate background of opera sung by the Diva during a performance. Picking up a case in the Diva's room that is implied to contain the stones, Leeloo is stopped by Zorg, who has boarded the ship to apprehend them for his own purposes. Armed with a high-powered gun, he coerces Leeloo into giving them to him. Tossing the case at him, sensing that the stones were in fact not inside it, Leeloo attempts to escape through the vents in the ceiling of the room. Zorg promptly opens fire, gravely injuring her. Leaving her to die, Zorg sets a bomb to detonate in the hotel in 20 minutes.

Leeloo remains in the vents, unable to escape due to her injuries and weakness. Korben, after seeing her hand hanging from the vents in security footage, races to her. He, Leeloo, Cornelius, and Ruby Rhod board the ship Zorg arrived on and escape the hotel before it explodes.

The group arrives at the temple and meet up with David. The stones, which Korben recovered from the body of the slain Diva, are put in proper position. Leeloo is carried to the center, but she is at first unwilling to assist in saving the planet due to the lack of regard that humans seem to have for their lives, as evidenced by brutal war clips she watched during transport. Horrified by human brutality, she resigns herself to not protecting what humans seemed bent on destroying. Korben reminds her that beautiful things, like love, are worth preserving. Leeloo, who was built as a weapon and therefore was unfamiliar with love, pleads with Korben to tell her why he needed her. Korben submits, admitting that he was in love with her. Kissing passionately, Leeloo uses the power of the stones to create the Divine Light, which destroys the oncoming Great Evil just seconds before it destroys Earth.

Later, Korben and Leeloo recover from their mission in a special revitalizing chamber in the same New York laboratory where Leeloo was recreated. The lead scientist awkwardly explains to the visiting President that the two heroes cannot meet with him at this time because they are making love inside the chamber.

Appearance Edit

Leeloo's human form after being recreated by the laboratory is that of a Caucasian female in her mid-to-late twenties. She is described by Korben as being "a really big fare that he couldn't resist", being 5'9", having long legs, blue eyes, and great skin. Florin Despa

Dr. Florin Despa is on the faculty of the Department of Pharmacology and Nutritional Sciences since 2013. Prior to his arrival at The University of Kentucky, Dr. Despa was a faculty member in The Department of Pharmacology at The University of California, Davis (UC Davis). He joined the faculty at UC Davis in 2008, after completing his research training in biomedical sciences at The University of Chicago (2000-2008), where he studied molecular mechanisms underlying cell injury and cell repair. An important research focus in Despa’s lab when at UC Davis was to understand how chronic hyperglycemia may impair insulin and amylin secretory lines in pancreatic ?-cells and identifying consequences of this impairment on cardiovascular structure and function. Between 1998 and 2000, Dr. Despa was with The School of Natural Sciences at K. U. Leuven (Belgium) where he was awarded with a postdoctoral fellow of the Foundation for Scientific Research – Flanders for studying molecular interactions in nanosystems. He was also a postdoctoral fellow of The Research Board of the K. U. Leuven and a visiting scientist at The Forschungszentrum Juelich (2008) and at The Humboldt University Berlin (1999). Dr. Despa graduated from The Institute of Physics, Magurele-Bucharest (Romania) in 1997.

Research in F. Despa’s lab focuses on the pathological effects of amylin hypersecretion. Hyperamylinemia is common in obesity, pre-diabetic insulin resistance, diets and substance abuse and leads to amylin oligomerization and proteotoxicity in pancreatic islets. His lab was the first to show that hyperamylinemia also affects the structure and function of blood vessels, heart and brain in humans. Work in his lab integrates biochemical, physiological and pharmacological analyses with clinical data and in vivo phenotyping. The long term goals of his research are 1) to gain insight into the molecular basis by which hyperamylinemia predisposes to cardio-cerebro-vascular complications and 2) to develop therapies and preventive strategies for limiting amylin-mediated pathological effects.

The amount of medicine that you take depends on the strength of the medicine. Also, the number of doses you take each day, the time allowed between doses, and the length of time you take the medicine depend on the medical problem for which you are using the medicine.

For redness, itching, and swelling of the skin:

Adults: Apply to the affected area of the skin one to three times per day.

Children: Use and dose must be determined by your doctor.

Store Betnovate at room temperature, between 68 and 77 degrees F (20 and 25 degrees C). Store away from heat, moisture, and light. Keep Betnovate out of the reach of children and away from pets.

Do NOT use Betnovate if:

you are allergic to any ingredient in Betnovate

you are taking mifepristone

you have a systemic fungal infection

you are scheduled to have a smallpox vaccine

Some medical conditions may interact with Betnovate. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

Some medicines may interact with Betnovate. Tell your health care provider if you are taking any other medicines, especially any of the following:

Aprepitant, clarithromycin, cyclosporine, diltiazem, erythromycin, itraconazole, ketoconazole, or troleandomycin because side effects, such as adrenal gland or nervous system problems (eg, seizures), may occur

Barbiturates (eg, phenobarbital), carbamazepine, hydantoins (eg, phenytoin), lithium, or rifampin because they may decrease Betnovate's effectiveness

Aspirin, live vaccines, mifepristone, or ritodrine because the risk of their side effects may be increased by Betnovate.

Important safety information:

If you use Betnovate for an extended period of time, your body may not produce enough natural steroids for up to several months after you stop using it.
