Ilsatec

Ilsatec - General Information

Ilsatec is a proton pump inhibitor which prevents the stomach from producing acid. It is manufactured by TAP Pharmaceutical Products. Ilsatec has been marketed for many years and is one of several PPI's available.

Pharmacology of Ilsatec

Ilsatec, an acid proton-pump inhibitor similar to omeprazole, is used as an untiulcer drug in the treatment and maintenance of healing of duodenal or gastric ulcers, erosive and reflux esophagitis, NSAID-induced ulcer, Zollinger-Ellison syndrome, and Barrett's esophagus. Lansoprozole is active against Helicobacter pylori . The plasma elimination half-life of lansoprazole does not reflect its duration of suppression of gastric acid secretion. Thus, the plasma elimination half-life is less than two hours, while the acid inhibitory effect lasts more than 24 hours.

Ilsatec for patients

Lansoprazole is used short-term (4-8 weeks) to treat duodenal ulcer and erosive esophagitis. It may used long term to treat Zollinger-Ellison syndrome, a problem with too much acid being secreted. Lansoprazole capsules should be taken before eating. The capsules are delayed release' meaning they work over time. The capsules should be swallowed whole and not crushed, opened or chewed. If you are taking theophylline, you may need your dosage checked when you start and stop lansoprazole to ensure your dose is effective. If you are taking sucralfate, it should be taken 30 minutes after taking lansoprazole. The most common side effects reported were diarrhea, nausea and abdominal pain. These occurred in fewer than 5% of patients. If your symptoms return after completing your course of therapy, talk to your physician for further evaluation.

Ilsatec Interactions

Lansoprazole is metabolized through the cytochrome P 450 system, specifically through the CYP3A and CYP2C19 isozymes. Studies have shown that lansoprazole does not have clinically significant interactions with other drugs metabolized by the cytochrome P 450 system, such as warfarin, antipyrine, indomethacin, ibuprofen, phenytoin, propranolol, prednisone, diazepam, clarithromycin, or terfenadine in healthy subjects. These compounds are metabolized through various cytochrome P 450 isozymes including CYP1A2, CYP2C9, CYP2C19, CYP2D6, and CYP3A. When lansoprazole was administered concomitantly with theophylline (CYP1A2, CYP3A), a minor increase (10%) in the clearance of theophylline was seen. Because the small magnitude and the direction of the effect on theophylline clearance, this interaction is unlikely to be clinical concern. Nonetheless, individual patients may require additional titration of their theophylline dosage when lansoprazole is started or stopped to ensure clinically effective blood levels.

Lansoprazole has also been shown to have no clinically significant interaction with amoxicillin.

In a single-dose crossover study examining lansoprazole 30 mg and omeprazole 20 mg each administered alone and concomitantly with sucralfate 1 gram, absorption of the proton pump inhibitors was delayed and their bioavailability was reduced by 17% and 16%, respectively, when administered concomitantly with sucralfate. Therefore, proton pump inhibitors should be taken at least 30 minutes prior to sucralfate. In clinical trials, antacids were administered concomitantly with lansoprazole delayed-release capsules; this did not interfere with its effect.

Lansoprazole causes a profound and long lasting inhibition of gastric acid secretion; therefore, it is theoretically possible that lansoprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability ( e. g. ketoconazole, ampicillin esters, iron salts, digoxin).

Ilsatec Contraindications

Lansoprazole is contraindicated in patients with known hypersensitivity to any component of the formulation.

Lansoprazole delayed-release capsules are contraindicated in patients with known hypersensitivity to any component of the formulation.

Amoxicillin is contraindicated in patients with a known hypersensitivity to any penicillin. (Please refer to full prescribing information for amoxicillin before prescribing.)

Clarithromycin is contraindicated in patients with a known hypersensitivity to any macrolide antibiotic, and in patients receiving terfenadine therapy who have preexisting cardiac abnormalities or electrolyte disturbances. (Please refer to clarithromycin before prescribing.)

Additional information about Ilsatec

Ilsatec Indication: For treatment of Acid-reflux disorders (GERD), peptic Ulcer Disease, duodenal ulcers, esophagitis, and Zollinger-Ellison syndrome Mechanism Of Action: Ilsatec belongs to a class of antisecretory compounds, the substituted benzimidazoles, that do not exhibit anticholinergic or histamine H 2 - receptor antagonist properties, but rather suppress gastric acid secretion by specific inhibition of the (H +,K + )-ATPase enzyme system at the secretory surface of the gastric parietal cell. Because this enzyme system is regarded as the acid (proton) pump within the parietal cell, Ilsatec has been characterized as a gastric acid-pump inhibitor, in that it blocks the final step of acid production. This effect is dose-related and leads to inhibition of both basal and stimulated gastric acid secretion irrespective of the stimulus. Drug Interactions: Atazanavir This gastric pH modifier decreases the levels/effects of atazanavir Enoxacin The agent decreases the absorption of enoxacin Indinavir Omeprazole decreases the absorption of indinavir Itraconazole The proton pump inhibitor decreases the absorption of imidazole Ketoconazole The proton pump inhibitor decreases the absorption of imidazole Sucralfate Sucralfate decreases the effect of lansoprazole Food Interactions: Avoid alcohol. Take 30-60 minutes before meals. Food reduces bioavailabilty, but this has very little clinical impact. Generic Name: Lansoprazole Synonyms: Not Available Drug Category: Anti-Ulcer Agents; Anti-Infectives; Proton-pump Inhibitors Drug Type: Small Molecule; Approved Other Brand Names containing Lansoprazole: Agopton; Amarin; Aprazol; Bamalite; Biuret; Biuret Gr; Biuret Reagent; Biuret Reagent Solution; Blason; Compraz; Dakar; Ilsatec; Ketian; Lancid; Lanproton; Lansopep; Lansoprazol [Inn-Spanish]; Lansoprazole [Usan-Ban-Inn]; Lansoprazolum [Inn-Latin]; Lanston; Lanz; Lanzol-30; Lanzopral; Lanzor; Lasoprol; Limpidex; Mesactol; Monolitum; Ogast; Ogastro; Opiren; Prevacid; Prevacid Iv; Prevacid Solutab; Prevpac; Prezal; Pro Ulco; Promp; Prosogan; Suprecid; Takepron; Ulpax; Zoprol; Zoton; Absorption: The absorption of lansoprazole is rapid, with mean C max occurring approximately 1.7 hours after oral dosing, and relatively complete with absolute bioavailability over 80%. Toxicity (Overdose): Symptoms of overdose include abdominal pain, nausea and diarrhea. Protein Binding: 97% Biotransformation: Hepatic. Two metabolites have been identified in measurable quantities in plasma (the hydroxylated sulfinyl and sulfone derivatives of lansoprazole). These metabolites have very little or no antisecretory activity. Lansoprazole is thought to be transformed into two active species which inhibit acid secretion by (H +,K + )-ATPase within the parietal cell canaliculus, but are not present in the systemic circulation. Warrant Definition, Warfant

Warrant

What is a 'Warrant'

A warrant is a derivative that confers the right, but not the obligation, to buy or sell a security – normally an equity – at a certain price before expiration. The price at which the underlying security can be bought or sold is referred to as the exercise price or strike price. An American warrant can be exercised at any time on or before the expiration date, while European warrants can only be exercised on the expiration date. Warrants that confer the right to buy a security are known as call warrants ; those that confer the right to sell are known as put warrants.

BREAKING DOWN 'Warrant'

Warrants are in many ways similar to options. but a few key differences distinguish them. Warrants are generally issued by the company itself, not a third party, and they are traded over-the-counter more often than on an exchange. Investors cannot write warrants like they can options. Unlike options (with the exception of employee stock options ), warrants are dilutive. when an investor exercises her warrant, she receives newly issued stock, rather than already-outstanding stock. Warrants tend to have much longer periods between issue and expiration than options, of years rather than months.

Warrants do not pay dividends or come with voting rights. Investors are attracted to warrants as a means of leveraging their positions in a security, hedging against downside (for example, by combining a put warrant with a long position in the underlying stock) or exploiting arbitrage opportunities.

Warrants are no longer very common in the U. S. but are heavily traded in Hong Kong, Germany and other countries.

Types Of Warrants

Traditional warrants are issued in conjunction with bonds. which in turn are called warrant-linked bonds, as a kind of sweetener that allows the issuer to offer a lower coupon rate. These warrants are often detachable. meaning that they can be separated from the bond and sold on the secondary markets before expiration. A detachable warrant can also be issued in conjunction with preferred stock ; often the warrant must be sold before the investor can collect dividends.

Wedded or wedding warrants are not detachable, and the investor must surrender the bond or preferred stock the warrant is "wedded" to in order to exercise it. Naked warrants are issued on their own, without accompanying bonds or preferred stock.

Covered warrants are issued by financial institutions rather than companies, so no new stock is issued when covered warrants are exercised. Rather, the warrants are "covered" in that the issuing institution already owns the underlying shares or can somehow acquire them. The underlying securities are not limited to equity, as with other types of warrants, but may be currencies, commodities or any number of other financial instruments.

Trading Warrants

Trading warrants can be difficult and time-consuming, as they are for the most part not listed on stock exchanges, and data on warrant issues is not readily available for free. When a warrant is listed on an exchange, its ticker symbol will often be the symbol of the company's common stock with a W added to the end. For example, Abeona Therapeutics Inc's (ABEO ) warrants are listed on Nasdaq under the symbol ABEOW. In other cases, a Z will be added, or a letter denoting the specific issue (A, B, C…).

Warrants generally trade at a premium. which is subject to time decay as the expiration date nears. As with options, warrants can be priced using the Black-Scholes model.

Ecoflox-Oz (Ofloxacin, Ecoflox

Medication: Ecoflox-OZ

Ecoflox-OZ consists of Ofloxacin . Ornidazole .

Ofloxacin - A synthetic fluoroquinolone (fluoroquinolones) antibacterial agent that inhibits the supercoiling activity of bacterial DNA gyrase, halting DNA replication.

Indication: For the treatment of infections (respiratory tract, kidney, skin, soft tissue, UTI), urethral and cervical gonorrhoea.

Ecoflox-OZ (Ofloxacin) is a quinolone/fluoroquinolone antibiotic. Ecoflox-OZ (Ofloxacin) is bactericidal and its mode of action depends on blocking of bacterial DNA replication by binding itself to an enzyme called DNA gyrase, which allows the untwisting required to replicate one DNA double helix into two. Notably the drug has 100 times higher affinity for bacterial DNA gyrase than for mammalian. Ecoflox-OZ (Ofloxacin) is a broad-spectrum antibiotic that is active against both Gram-positive and Gram-negative bacteria.

Exercise And Physical Therapy, Parkisan

Research has shown that regular exercise benefits people with Parkinson’s disease .

reduces stiffness

improves mobility, posture, balance and gait

Aerobic exercise increases oxygen delivery and neurotransmitters to keep our heart, lungs, and nervous system healthy. General exercise may also reduce depression . Learning-based memory exercises can also help keep our memory sharp (Positscience. Lumosity ).

Click on question below to jump ahead to a particular answer or scroll down to read each in order.

What types of exercise are best for people with Parkinson’s disease?

There is increasing evidence that aerobic and learning-based exercises could be neuroprotective in aging individuals and those with neurodegenerative disease. Facilitating exercise programs that challenge our heart and lungs as well as promote good biomechanics, good posture, trunk rotation and normal rhythmic, symmetric movements are the best. Dancing to music may be particularly good for decreasing stiffness.

Although research on this subject is ongoing, it does appear that beyond aerobic activities performed with healthy movement patterns, exercises challenging the individual to change tempo, activity, or direction (what is referred to as “random practice” exercise) benefits people with Parkinson’s disease. It is also important to keep variety in exercise activities, because individuals with Parkinson’s disease often have difficulty in shifting from one activity to another or in performing two activities at the same time. Exercises that require balance and preparatory adjustment of the body are also important along with rhythmic activities such as dancing, skipping and cycling can maintain the ability to perform reciprocal movements. Finally, exercises that promote attention and learning are beneficial.

Walking outside or in a mall

Dancing

Yoga classes

Tai Chi classes

Stepping over obstacles

Marching to music with big arm swings

Sports (ping pong, golf, tennis, volleyball)

Aerobic/Jazzercise classes

Types of exercises that promote cardiopulmonary fitness :

Paced walking (treadmill walking at different speeds and different inclines)

Hiking using walking sticks

Swimming with different strokes with the eyes open and closed (+)not only challenge motor learning but also increase heart rate and provide good cardiopulmonary conditioning.

New bodyweight-supported treadmills can also be helpful to protect from falling, and to facilitate easier coordinated movements for fast walking with a long stride or jogging.

Types of exercise that do NOT challenge motor planning :

Riding a stationary bicycle without doing other activities

Weight lifting

Treadmill walking at a slow speed

Lap swimming can be very habitual and also automatic.

These exercises for cardiovascular, endurance and strengthening could be enriched by performing simultaneous activities such as reading, writing, problem solving, singing, watching the news or a movie or throwing and catching balls. Exercises that demand attention, repetition, progression of difficulty with spaced practice over time are the best exercise routines to promote learning.

You can purchase learning programs from Positscience and Lumosity. Currently, Dr. Dowling and Dr. Melnick in collaboration with Red Hills Studio are developing fun motor learning programs that can improve posture and balance. The Wii sports games and balance activities may be generally fun and helpful (Wii Study at UCSF ).

Is there any value in strength training?

Weight lifting per se is NOT the best choice of an exercise program for the person with Parkinson’s disease, particularly if it is the only exercise activity. Individuals need to be careful how they perform strengthening exercises to minimize increasing stiffness and rigidity. When performed properly . strengthening exercises do have some value .

As one ages . more exercise must be performed to maintain muscle mass. Muscle mass and strength allow an individual to complete daily chores and to maintain balance. Additionally, strengthening postural muscles may help to maintain a more upright posture. Integrative, functional exercises other than weight-training may strengthen muscles in ways that are more beneficial to individuals with Parkinson’s disease.

Examples of alternative exercises to weight lifting :

Activities in a standing position strengthen legs

Pushing up to rise on the toes

Modified squats

Repetitively rising and sitting from a chair

Wearing ankle and wrist weights around the house or out on a walk

Push-ups or wall push-ups for arms

Light weights are just as effective as heavy weights in maintaining muscle tone and do not increase stiffness as much. Walking with ankle and wrist weights can help strengthen while encouraging increased awareness of arm swinging and high stepping. Moderation is the best word for strength training without other forms of exercise. However, integrating strengthening and flexibility exercises into aerobic, rhythmic and learning-based exercise routines that are fun, engaging, progressing in difficulty and rewarding are the best.

Swimming provides good cardiopulmonary training and maintains muscle strength . However . lap swimming does not challenge balance or stimulate variety of movements. Therefore, lap swimming is a second-choice activity . However, since the arms, legs and head may be doing different things, it may increase coordination. The resistance of the water increases stiffness in some people and decreases it in others.

Adding resistance with paddles and trunk support – provides more opportunity for reciprocal movements and circling movements of the arms and the legs

Rolling and somersaults – in the pool are good for those who are particularly comfortable in the water

For individuals with Parkinson’s disease who have difficulty in breathing, swimming may not be a comfortable aerobic activity. Thus, swimming may be an appropriate choice of exercise for individuals who have enjoyed it in the past and are comfortable with the techniques and those with musculoskeletal conditions particularly of the knee and back. Swimming using certain strokes can also help increase shoulder range of motion.

Will exercise make my muscles less stiff?

Exercises that require large, rhythmical movements through a full range of motion have been shown to decrease rigidity . For example, in a program of aerobic exercise using music, there was a reduction in rigidity in 9 out of 10 participants immediately after the exercise program.

Exercises to decrease stiffness :

Large, rhythmical movements

Rotating the trunk

Vibration, rocking and swinging

Other considerations to decrease stiffness :

Avoiding tremors (e. g. touching the limb that is shaking to quiet the movement) can also decrease tension

Decreasing stress in ones life – having fun, thinking positively about planning and carrying out challenging, socially engaging and learning-based activities

Cooling or warming the tense extremity can sometimes be helpful.

When should I exercise in relation to medication?

The best time to exercise is when mobility is best . For individuals who take medications for Parkinson’s disease, the best level of function often occurs about 1 hour after a dose of medications. The answer to this question varies by individual . The individual reaction to the medication is also important.

How often should I exercise?

The guidelines for people with Parkinson’s disease are no different from those without the disease (i. e. 4-5 times a week for at least 30-40 minutes ). This assumes that your heart is beating at 70 to 80% of maximum (220 – your age times 70 or 80%).

Make the exercise time fun :

Engage in group exercise, movement or dancing classes. For many, participating in activities with other people, can be more stimulating and increase compliance.

Stay active and integrate exercise into your usual day :

Walk whenever possible instead of driving

Climb the stairs instead of taking the elevator

Take regular 5 minute breaks every 30 minutes (lifting the arms up over your head, performing wall glides, breathing diaphragmatically, getting up to get a glass of water, or putting theraband on chairs to work on some strengthening)

Avoid long periods of time watching TV and or using a computer

Is there anything else I should know?

A “cool-down period” is important . After exercise, allow yourself a longer time for a cool-down than others would need (Individuals who exercised before developing Parkinson’s disease typically double their cool-down time).

A cool-down period accomplishes 2 goals :

1) Promotes a slow decrease in heart rate

2) Allows the muscles time to cool down gradually so they do not become stiff.

A cool-down period consists of the same exercise activity but at a progressively slower pace. During the cool-down, all muscles need to go through a slow, full range of motion. If you feel exhausted and want to fall asleep immediately after exercise, then you are not cooling down slowly enough.

Learn something new every day .

If you listen to the news, talk to someone about it. Listen to educational programs and discuss what you learned. Do crossword puzzles or participate in memory training programs on the web or from a CD.

Challenge yourself to go out each day .

If you are retired, consider volunteering your time to help others ( e. g. Red Cross, Meals on Wheels). Move about in the community and learn the tricks of keeping your eyes on a target to improve stability. Carry a cane to let people know that it would be best not to bump you.

Learn to hold your pen lightly and write with big cursive type movements. Consider making the surface of your pen rough or sticky. This will help decrease the force of your squeezing the pen. Write by moving the whole arm, not just the fingers. Practice writing to music and even say the words out loud as you write. Circle making big movements. Get a drawing pad from a toy store where your can lift up the writing surface and erase your practice strokes.

Exercise your voice .

Talk slowly, clearly and loudly with a lot of expression of your eyes and your face. Have everyone speak loudly and slowly. If you find you continue to talk softly and quickly and people are having difficulty understanding you, then ask a friend to read and record some passages from a book. Then you put the head set on and hear your friends voice as you read the same passages. This may strengthen the learning.

When should I request a referral for Physical Therapy?

When first diagnosed . all patients should have a consultation with a physical therapist to define the appropriate exercise program tailored to “you”. This will also establish a baseline of your current physical status.

Ideally, all patients with PD should have a good fitness program as well as specific exercises to maintain good posture and balance as well as improve symmetry in flexibility and strength. Then, one may benefit from a consultation with a physical therapist when signs and symptoms increase the risk for falling or limit comfortable community mobility and confidence. The therapist will also work on improving gait with practice using visual and auditory cues, as well as without those cues.

As the disease progresses, periodic re-evaluations are helpful to assure your exercise program is having the maximum benefit. A program of individualized exercises addressing posture, balance and gait has been shown to be beneficial in decreasing the risk of falling. In some cases, where balance or musculoskeletal problems develop, supervised outpatient treatments a few times per week may be helpful for a few weeks.

Safe mobility may be enhanced by using a bodyweight supported treadmill to minimize discomfort, assure stability, decrease the fear of falling and facilitate normal movement. (Available with Dr. Nancy Byl's Neurofit program at UCSF Mission Bay)

In addition to physical exercise, it is important for everyone to maintain a high level of learning-based exercise activities to improve the ability to do multiple tasks simultaneously and safely without falling. The combination of learning-based memory training, aerobic exercise, over ground gait practice and integrative, engaging activities in the community are essential to positive health and well being despite aging or neurodegenerative disease.

Are there techniques to help me walk?

Often individuals with Parkinson’s disease have problems with abruptly halting or “ freezing ,” when walking.

To help decrease freezing, try :

Reciprocal arm swinging

High long steps

Scanning the environment and using visual fixation on an object in the distance or auditory cues (listening to music, singing to yourself, counting)

Thinking about making big steps to clear obstacles on the floor or marching (high steps)

Walking hand-in-hand, swinging the arms with a friend or family member

Having someone place their foot in front of you as a cue to step high and over

One person found that throwing pennies and stepping over them was helpful (“But,” he added, “don’t bend down to pick them up.”).

Loud rhythmical clapping

Paced walking with high stepping.

Using walking sticks (using them for sensory feedback and sense of stability may be more important than using a cane. Of course, using a cane or a walker can be helpful if there is a lot of weakness and stiffness.)

Pushing one’s self to stay active should be the goal .

Are there hints to help me get out of bed?

For some people with Parkinson’s disease, getting out of bed may become difficult. First, you might practice rolling on your bed. Roll back and forth and get a rhythm. Then with some momentum, roll to your side and come to sitting. You might have a dresser near the side of your bed so you could touch the dresser for stability if you need to.

When specifically trying to come to sitting from lying in bed, roll over on your side. Push on your elbow and let your feet come over the edge of the bed and then sit up at the edge. Sit there for a minute to adjust and then rise to stand and then walk. This technique is not only easier but better for your back.

Techniques to make it easier to move around in bed :

Wear satin pajamas or use satin sheets (but not both).

Practice standing up and sitting down quickly from a chair without using your handsto improve your transitional movement skills

Practice getting up quickly from a chair and taking a few big steps and then turn around and go sit back down.

Practice safe skills by climbing and descending stairs.

If you are seeing a physical therapist, the last three bullets are some of the activities that should be practiced.

Frequently patients with Parkinson’s disease have one limb or one side that is more involved. It is easy to stop using the more involved side because it is too difficult. This leads to overuse of the lesser affected side and neglect of the more affected side. This disuse of the affected limb can lead to a worsening of the signs and symptoms. There is evidence that constraining the least affected side, and forcing the use of the more affected side can lead to improved function and increased “mindfulness” of the limb.

Forced use and constraining the least affected side has been effective in driving neural adaptation. This has been demonstrated in animal studies with drug induced PD. It has also been demonstrated in patients post-stroke. This forced use can be enhanced not only by physical practice, but mental practice as well. Each day, time should be spent on strengthening, range of motion, task performance and coordination exercises of the more involved limb. Force yourself to do as many tasks as possible with the involved upper limb, using the other limb only to help stabilization. In addition, do some drills like tap the fingers, tap the wrist and then tap the forearm moving from the elbow as fast as possible. You can also practice turning the palm up and down as fast as possible, throwing and catching balls, putting small objects in small containers, taking your index finger to quickly touch objects that are moving those that and still. Do similar activities with the leg.

This principle of targeting specific task-oriented use of an extremity is similar to forcing yourself to exercise at an intense versus a low level. ‘Forced,’ in this context, means that you work harder than you would usually work. In other words, in your general exercise routine, you want to work with a faster speed and potentially for a longer period to keep your nervous system and your cardiovascular system adaptable and responsive. This will also allow your nervous system to respond more crisply when faced with unexpected and surprise events. This will also facilitate better balance responses and improved postural alignment.

Overview

This drug is used by mouth or rectally to treat or prevent complications of liver disease (hepatic encephalopathy). It does not cure the problem, but may help to improve mental status. Lactulose is a colonic acidifier that works by decreasing the amount of ammonia in the blood. It is a man-made sugar solution.

How to use

If you are taking this medication by mouth for liver disease, take it usually 3-4 times a day or as directed by your doctor. To improve the taste, you may mix it into fruit juice, water, milk, or a soft dessert. The goal is to have 2-3 soft stools each day. Dosage is based on your medical condition and response to therapy (i. e. the number of soft stools each day).

If you are taking this medication by mouth for constipation, take it usually once daily or as directed by your doctor.

This medication can also be given rectally as an enema for liver disease. Mix the recommended amount of lactulose with 700 milliliters (24 ounces) of water or normal saline. Give the solution into the rectum and keep the liquid inside for 30-60 minutes as directed by your doctor. If you keep the enema inside for less than 30 minutes, repeat the dose unless directed otherwise.

Use this medication regularly in order to get the most benefit from it. Remember to use it at the same time each day.

When this drug is given rectally for liver disease, an improvement in your mental status may occur in as little as 2 hours, but if you are taking this medication by mouth, it may take up to 24 to 48 hours. If you are using this medication for constipation, it may take up to 48 hours to have a bowel movement. Inform your doctor if your condition persists or worsens.

Side effects

Gas, bloating, burping, stomach rumbling/pain, nausea, and cramps may occur. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Tell your doctor right away if any of these unlikely but serious side effects occur: diarrhea, vomiting, muscle cramps/weakness, irregular heartbeat, mental/mood changes, seizures.

A very serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction may include: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Estradiol Gel Indications, Side Effects, Warnings, Oestrodose

Estradiol gel

Generic Name: estradiol (ES-tra-DYE-ol) Brand Name: Divigel

Estradiol gel should not be used to prevent heart disease, heart attacks, strokes, or dementia. Estrogens have been shown to increase the risk of heart disease (including heart attack), stroke, dementia, serious blood clots (eg, in the lungs or legs), cancer of the uterus, and breast cancer in some women. Tell your doctor right away if you have unusual vaginal bleeding while you use estradiol gel. Talk with your doctor if you have questions about the benefits and risks of using estradiol gel.

Estradiol gel should be used for the shortest possible time at the lowest effective dose to minimize the risk of these side effects. Talk with your doctor regularly about your need to use estradiol gel.

Estradiol gel is used for:

Treating certain moderate to severe symptoms of menopause (eg, hot flashes).

Estradiol gel is an estrogen hormone. It works by replacing estrogen in the body when it no longer produces enough on its own.

Do NOT use estradiol gel if:

you are allergic to any ingredient in estradiol gel

you have had a severe reaction (eg, swelling of the hands, face, lips, eyes, throat, or tongue; trouble swallowing or breathing; hoarseness) after taking estradiol gel

you are pregnant or think you may be pregnant

you have undiagnosed abnormal vaginal bleeding

you have known or suspected breast cancer or another estrogen-dependent cancer, or you have a history of breast cancer

you have liver problems or liver disease

you have active blood clots (eg, of the legs or lungs), a history of blood clots, or a condition that may increase your risk of blood clots

you have had a heart attack or stroke within the past 12 months

Contact your doctor or health care provider right away if any of these apply to you.

Before using estradiol gel:

Some medical conditions may interact with estradiol gel. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

if you are pregnant, planning to become pregnant, or are breast-feeding

if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

if you have allergies to medicines, foods, or other substances

if you have a condition called hereditary angioedema

if you have a history of breast lumps, breast disease, or an abnormal mammogram, or if a member of your family has had breast cancer

if you have a vaginal infection or a history of uterus problems (eg fibroids, endometriosis, abnormal vaginal bleeding, cancer), or you have had your uterus removed (hysterectomy)

if you have a history of asthma, depression, memory problems, diabetes, seizures, gallbladder disease, pancreatitis, heart problems, heart attack, stroke, high blood cholesterol or triglyceride levels, high blood calcium levels, high blood pressure, low blood levels of parathyroid hormone, or porphyria

if you have a history of kidney or liver problems, yellowing of the skin or eyes, cancer, lupus, migraines, thyroid problems, or chorea (jerky, involuntary movements of the face, arms, or legs)

if you smoke, use tobacco, are very overweight, will be having surgery, or will be confined to a bed or chair for a period of time

if a member of your family has had high cholesterol or triglyceride levels, blood clots (eg, in the leg or lung), or lupus, or has been very overweight

Some MEDICINES MAY INTERACT with estradiol gel. Tell your health care provider if you are taking any other medicines, especially any of the following:

Azole antifungals (eg, ketoconazole), macrolide antibiotics (eg, erythromycin), or protease inhibitors (eg, ritonavir) because they may increase the risk of estradiol gel's side effects

Barbiturates (eg, phenobarbital), carbamazepine, hydantoins (eg, phenytoin), rifamycins (eg, rifampin), or St. John's wort because they may decrease estradiol gel's effectiveness

Thyroid hormones (eg, levothyroxine) because their effectiveness may be decreased by estradiol gel

This may not be a complete list of all interactions that may occur. Ask your health care provider if estradiol gel may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.

How to use estradiol gel:

Use estradiol gel as directed by your doctor. Check the label on the medicine for exact dosing instructions.

An extra patient leaflet is available with estradiol gel. Talk to your pharmacist if you have questions about this information.

Apply estradiol gel to the upper thigh area of the right or left leg. Change between your left and right thigh each day to help avoid skin irritation.

Apply estradiol gel to clean, intact skin only. Be sure the area is completely dry before applying estradiol gel.

Do not apply estradiol gel to skin that is irritated or broken. Do not apply it to your face, in or around the vagina, or to your breasts.

If you take a bath or shower, or use a sauna around the time of your dose, apply estradiol gel afterwards. Be sure the area is completely dry before applying the medicine.

Open the packet and squeeze the entire contents of the packet onto the upper thigh of one leg. Gently spread the medicine as thinly as possible over an area of about 5 to 7 inches (about 2 palm prints). You do not need to rub the medicine in.

Throw away the empty packet in the trash out of the reach of children and pets.

Wash your hands with soap and water after using estradiol gel.

Cover the application site with clothing after the medicine is completely dry. This will help to prevent others from coming into contact with the medicine. Be sure the medicine is completely dry before you dress.

Do not wash the application site for at least 1 hour after you apply estradiol gel.

Do not allow others to apply estradiol gel for you. Do not allow others to come into contact with the application site for at least 1 hour after you apply estradiol gel. If someone else comes into contact with estradiol gel, have them wash the area with soap and water right away.

Use estradiol gel at the same time each day.

If you spill some of estradiol gel while you are applying it, do not open a new packet. Wait and apply your regular dose the next day.

If you miss a dose of estradiol gel, use it as soon as possible. If your next dose is less than 12 hours away, skip the missed dose and go back to your regular dosing schedule. Do not use 2 doses at once.

Ask your health care provider any questions you may have about how to use estradiol gel.

Important safety information:

Estradiol gel may increase the risk of stroke, heart attack, blood clots, high blood pressure, or similar problems. The risk may be greater if you smoke.

Estradiol gel is for external use only. Do not get it in your eyes, nose, vagina, or mouth. If you get it in any of these areas, rinse right away with warm clean water.

Check with your doctor before applying sunscreen to the application site while using estradiol gel.

If you will be swimming, wait until estradiol gel is completely dry before going into the water.

Estradiol gel is flammable. Do not store or use near heat or an open flame. Avoid fire, flame, or smoking until the medicine has dried on your skin.

Eating grapefruit or drinking grapefruit juice may increase the risk of estradiol gel's side effects. Talk to your doctor before including grapefruit or grapefruit juice in your diet while you are taking estradiol gel.

Tell your doctor or dentist that you take estradiol gel before you receive any medical or dental care, emergency care, or surgery. If you will be having surgery or will be confined to a chair or bed for a long period of time (eg, a long plane flight), notify your doctor at least 4 to 6 weeks beforehand. You may need to stop taking estradiol gel or take other special precautions for a period of time.

Estradiol gel may cause dark skin patches on your face. Exposure to the sun may make these patches darker. If patches develop, use a sunscreen or wear protective clothing when exposed to the sun, sunlamps, or tanning booths.

If you wear contact lenses and you develop problems with them, contact your doctor.

Diabetes patients - Estradiol gel may affect your blood sugar. Check blood sugar levels closely. Ask your doctor before you change the dose of your diabetes medicine.

Estradiol gel may interfere with certain lab tests. Be sure your doctor and lab personnel know you are using estradiol gel.

Temporary relief of cough and upper respiratory tract symptoms, including nasal congestion, associated with allergy or common cold.

this medicine is contraindicated in individuals with known hypersensitivity to paracetamol or any other constituents of the product. this medicine is contraindicated in patients with severe hypertension or coronary artery disease. this medicine is contraindicated in patients who are taking or have taken monoamine oxidase inhibitors within the preceding two weeks. The concomitant use of pseudoephedrine and this type of product may occasionally cause a rise in blood pressure. Not to be used in children under the age of 12 years.

Pseudoephedrine Serious side effects associated with the use of pseudoephedrine are extremely rare. Symptoms of central nervous system excitation may occur, including sleep disturbance and, rarely, hallucinations. Skin rashes, with or without irritation, have occasionally been reported with pseudoephedrine. Urinary retention has been reported occasionally in men receiving pseudoephedrine: prostatic enlargement could have been an important predisposing factor. Paracetamol Adverse effects of paracetamol are rare but hypersensitivity including skin rash may occur. There have been reports of blood dyscrasias including thrombocytopenia and agranulocytosis, but these were not necessarily causality related to paracetamol.

Concomitant use of this medicine with tricyclic antidepressants, sympathomimetic agents (such as decongestants, appetite suppressants and amfetamine-like psychostimulants) or with monoamine oxidase inhibitors, which interfere with the catabolism of sympathomimetic amines, may occasionally cause a rise in blood pressure. Because of the pseudoephedrine content, this medicine may partially reverse the hypotensive action of drugs which interfere with sympathetic activity including bretylium, betanidine, guanethidine, debrisoquine, methyldopa, alpha - and beta-adrenergic blocking agents. The use of drugs which induce hepatic microsomal enzymes, such as anticonvulsants and oral contraceptive steroids, may increase the extent of metabolism of paracetamol, resulting in reduced plasma concentrations of the drug and a faster elimination rate. The speed of absorption of paracetamol may be increased by metoclopramide or domperidone and absorption reduced by colestyramine. The anticoagulant effect of warfarin and other coumarins may be enhanced by prolonged regular use of paracetamol with increased risk of bleeding; occasional doses have no significant effect.

Although pseudoephedrine has virtually no pressor effects in normotensive patients, this medicine should be used with caution in patients suffering from mild to moderate hypertension. As with other sympathomimetic agents this medicine should be used with caution in patients with heart disease, diabetes, hyperthyroidism, elevated intraocular pressure or prostatic enlargement. Caution should be exercised in the administration of this medicine to patients with severe hepatic impairment or moderate to severe renal impairment, (particularly if accompanied by cardiovascular disease). The hazards of overdose are greater in those with non-cirrhotic alcoholic liver disease. Concomitant use of other products containing paracetamol or decongestants with this medicine could lead to overdose and therefore should be avoided. The following statements will appear on packs of this product: Keep out of the reach and sight of children. As with all medicines, if you are pregnant or currently taking any other medicine, consult your doctor or pharmacist before taking this product. If symptoms persist consult your doctor. Store below 25?C. Keep dry. Protect from light. Warning: Do not exceed the stated dose. Causes no drowsiness. Contains paracetamol. The label contains the following additional statements: Immediate medical advice should be sought in the event of an overdose, even if you feel well. Do not take with any other paracetamol-containing products. Immediate medical advice should be sought in the event of an overdose, even if you feel well, because of the risk of delayed, serious liver damage (leaflet).

RANTIN

The National Theatre of Scotland, in association with the Arches, presents

By Kieran Hurley

Created with Liam Hurley, Gav Prentice, Julia Taudevin and Drew Wright.

A retired American steps off the plane at Prestwick, hoping to discover the land of his fathers.

An old drunk man in Methil has a mystical vision at the harbour.

A supermarket checkout girl in Port Glasgow approaches work with a golf club.

Set specifically in the town of each performance and drawing on storytelling, live music and the Scottish folk tradition, Rantin attempts to stitch together visions of Scotland’s past with its ever-changing present reality, revealing the patchwork identity of a nation.

Originally presented as part of the Auteurs project, a collaboration between National Theatre of Scotland and the Arches, and Behaviour 2013 with support from the Esmee Fairbairn Foundation, Rantin is part cosy living room gathering, part play, part gig session.

“…this gorgeous piece of ceilidh-theatre, where music swirls engagingly, is taut, nuanced and illuminating” (The Herald)

What design elements in my Ecwid store can I change?

Ecwid allows you to change CSS styles of customer storefront, including colors, fonts, margins, and images. This helps your integrated Ecwid store match the design of your site completely.

HTML is not available for editing, that is you can’t change the interface logic (how Ecwid behaves). We believe that most HTML changes people do are outside of Ecwid boundaries.

Ecwid does not need those "change header HTML / change footer HTML" functionality. The site owner can change the layout of the whole site. And CSS capabilities within Ecwid are more than enough for you to make Ecwid look as an integral part of your site.

In most cases, you won't have to edit anything, not even the CSS. Just pick one of the pre-defined schemes. Ecwid is designed to fit well in most sites.

If you’d like to change some elements in your store, our FAQs will guide you through this process and give you many ready-to-use solutions.

Ecwid can be integrated into any website, blog, or social network page. Or in several places at the same time: Ecwid is a wonderful solution to open multiple selling channels for your business.

Facebook. Joomla. WordPress. Wix - these are just some of the most popular platforms or site-builders that offer built-in plugins for Ecwid. You can find setup instruction for many other website platforms in our help center . If you have a custom web site, you can embed Ecwid there as well. It is possible to add Ecwid to any site that allows embedding HTML/JavaScript code. All you need is:

Create a free Ecwid account at my. ecwid. com.

Get the integration code through the setup wizard on Dashboard page in your Ecwid control panel or generate it here .

Copy the integration code into your web-page.

What requirements does Ecwid have for hosting?

Ecwid is a SaaS solution which stands for Software as a Service. In short, this means that your Ecwid shop and all its data is stored and processed on our own servers (we use Amazon Web Services for that). Ecwid does not use or consume resources of the server that runs your website.

The main advantage of this approach is that all software updates/upgrades/fixes are delivered automatically to your Ecwid store by our team. Security of your data is also our major daily concern and is handled by us as well. We take care of all the technical aspects so that you could focus on selling online.

You can use Ecwid with any site/hosting that allows embedding HTML/Javascript code (and even open an online store if you don't have a website/hosting with our Starter Site yet). So you should be choosing the hosting based on the requirements of your website's engine and other services that you are going to use and run on your webpages.

How will I receive payments from my customers?

Ecwid provides several ways to process payments. You can setup your Ecwid store to receive online payments like PayPal and/or credit cards (through various payment processors) as well as simply arrange the payment offsite. Check out this topic explaining this question in detail: Payment Options

Can I set the tax rate for a certain area or state?

Yes. You can assign a tax rate for a country, state, or ZIP code. When a customer checks out, we automatically apply the tax rate from their billing or mailing address. If you have multiple states or countries with the same tax rate, you can create a destination zone for that rate.

Can I have different combinations for the same product?

Yes. You can set different combinations for each product (size, color, etc), and assign a different image/price/SKU/quantity/low stock notification for each. Learn more about product combinations in this article .

Blood Pressure Buy Lasix At, Urever

Blood Pressure

Lasix (Urever)

Lasix is used for treating high blood pressure or water retention (swelling) associated with congestive heart failure, liver disease, or kidney disease. Lasix is a loop diuretic. Loop diuretics make the kidneys eliminate larger amounts of electrolytes (especially sodium and potassium salts) and water than normal (diuretic effect). Loop diuretics are useful for treating many conditions in which salt and water retention (eg, edema, swelling) are a problem.

Use Lasix as directed by your doctor.

Take Lasix by mouth with or without food.

If you take cholestyramine, colestipol, or sucralfate, ask your doctor or pharmacist how to take them with Lasix.

Lasix may increase the amount of urine or cause you to urinate more often when you first start taking it. To keep this from disturbing your sleep, try to take your dose before 6 pm.

If you miss a dose of Lasix, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use Lasix.

Store Lasix at room temperature, between 68 and 77 degrees F (20 and 25 degrees C). Store away from heat and moisture in a tight, light-resistant container. Exposure to light may cause a slight discoloration. Do not take discolored tablets. Do not store in the bathroom. Keep Lasix out of the reach of children and away from pets.

Active Ingredient: Furosemide.

Do NOT use Lasix if:

you are allergic to any ingredient in Lasix

you are unable to urinate

you are taking ethacrynic acid.

Contact your doctor or health care provider right away if any of these apply to you.

Some medical conditions may interact with Lasix. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

if you are pregnant, planning to become pregnant, or are breast-feeding

if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

if you have allergies to medicines, foods, or other substances

if you have had a heart attack, are dehydrated, or are on a low-salt diet.

Some medicines may interact with Lasix. Tell your health care provider if you are taking any other medicines, especially any of the following:

Barbiturates (eg, phenobarbital) or narcotics (eg, codeine) because the risk of dizziness upon standing may be increased

Aminoglycosides (eg, gentamicin), amphotericin B, angiotensin-converting enzyme (ACE) inhibitors (eg, captopril), cyclosporine, ethacrynic acid, tacrolimus, or vancomycin because serious side effects to the kidneys (decreased ability to urinate) or ears (hearing loss) may occur

Chloral hydrate because side effects, such as excessive sweating, rapid heartbeat, and changes in blood pressure, may occur

Nonsteroidal anti-inflammatory drugs (NSAIDs) (eg, ibuprofen, indomethacin) because they may decrease Lasix's effectiveness

Digoxin, lithium, medicines for high blood pressure, salicylates (eg, aspirin), or succinylcholine because the risk of their side effects may be increased by Lasix

Norepinephrine or tubocurarine because their effectiveness may be decreased by Lasix.

This may not be a complete list of all interactions that may occur. Ask your health care provider if Lasix may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.

Important safety information:

Lasix may cause dizziness or blurred vision. These effects may be worse if you take it with alcohol or certain medicines. Use Lasix with caution. Do not drive or perform other possible unsafe tasks until you know how you react to it.

Lasix may cause dizziness, lightheadedness, or fainting; alcohol, hot weather, exercise, or fever may increase these effects. To prevent them, sit up or stand slowly, especially in the morning. Sit or lie down at the first sign of any of these effects.

Diabetes patients - Lasix may affect your blood sugar. Check blood sugar levels closely. Ask your doctor before you change the dose of your diabetes medicine.

Tell your doctor or dentist that you take Lasix before you receive any medical or dental care, emergency care, or surgery.

Your doctor may also prescribe a potassium supplement while you take Lasix. Check with your doctor before you use a salt substitute or other product that has potassium in it.

Lasix is a strong diuretic. Using too much of this drug can lead to serious water and mineral loss. Therefore, it is important that you be monitored by your doctor. Promptly notify your doctor if you become very thirsty, have a dry mouth, become confused, or develop muscle cramps/weakness.

Patients who take medicine for high blood pressure often feel tired or run down for a few weeks after starting treatment. Be sure to take your medicine even if you may not feel "normal." Tell your doctor if you develop any new symptoms.

If you have high blood pressure, do not use nonprescription products that contain stimulants. These products may include diet pills or cold medicines. Contact your doctor if you have any questions or concerns.

Lasix may cause you to become sunburned more easily. Avoid the sun, sunlamps, or tanning booths until you know how you react to Lasix. Use a sunscreen or wear protective clothing if you must be outside for more than a short time.

Lab tests, including blood pressure and complete blood counts, may be performed while you use Lasix. These tests may be used to monitor your condition or check for side effects. Be sure to keep all doctor and lab appointments.

Use Lasix with caution in the elderly; they may be more sensitive to its effects, especially dehydration.

Caution is advised when using Lasix in children; they may be more sensitive to its effects.

Pregnancy and breast-feeding: If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of using Lasix while you are pregnant. Lasix is found in breast milk. If you are or will be breast-feeding while you use Lasix, check with your doctor. Discuss any possible risks to your baby.

All medicines may cause side effects, but many people have no, or minor, side effects.

Check with your doctor if any of these most common side effects persist or become bothersome:

Dizziness; lightheadedness; sensitivity to sunlight.

Seek medical attention right away if any of these severe side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); calf pain or tenderness; confusion; dark urine; decreased or persistent increased urination; drowsiness; dry mouth; fast or irregular heartbeat; fever, chills, or sore throat; hearing decrease or loss; muscle pain/cramps/weakness; restlessness; ringing in the ears; severe or persistent nausea, vomiting, or dizziness; shortness of breath; sluggishness; stomach pain; unusual bleeding or bruising; unusual thirst or hunger; unusual tiredness or weakness; vein inflammation; yellow vision; yellowing of the skin or eyes.

This is not a complete list of all side effects that may occur. If you have questions about side effects, contact your health care provider.

Cefzon Generic Name Cefdinir Online, Cefzon

Cefzon General Information

As with other cephalosporins, bactericidal activity of cefdinir results from inhibition of cell wall synthesis by acting on penicillin binding proteins (PBPs).

Cefzon for patients

Patients should be counseled that antibacterial drugs including OMNICEF should only be used to treat bacterial infections. They do not treat viral infections (e. g. the common cold). When OMNICEF is prescribed to treat a bacterial infection, patients should be told that although it is common to feel better early in the course of therapy, the medication should be taken exactly as directed. Skipping doses or not completing the full course of therapy may (1) decrease the effectiveness of the immediate treatment and (2) increase the likelihood that bacteria will develop resistance and will not be treatable by OMNICEF or other antibacterial drugs in the future.

Antacids containing magnesium or aluminum interfere with the absorption of cefdinir. If this type of antacid is required during OMNICEF therapy, OMNICEF should be taken at least 2 hours before or after the antacid.

Iron supplements, including multivitamins that contain iron, interfere with the absorption of cefdinir. If iron supplements are required during OMNICEF therapy, OMNICEF should be taken at least 2 hours before or after the supplement.

Iron-fortified infant formula does not significantly interfere with the absorption of cefdinir. Therefore, OMNICEF for Oral Suspension can be administered with iron-fortified infant formula.

Diabetic patients and caregivers should be aware that the oral suspension contains 2.86 g of sucrose per teaspoon.

This description is suitable for active ingredient Cefdinir

Cefzon Interactions

Antacids (aluminum - or magnesium-containing)

Concomitant administration of 300-mg cefdinir capsules with 30 mL Maalox ® TC suspension reduces the rate (C max ) and extent (AUC) of absorption by approximately 40%. Time to reach C max is also prolonged by 1 hour. There are no significant effects on cefdinir pharmacokinetics if the antacid is administered 2 hours before or 2 hours after cefdinir. If antacids are required during OMNICEF therapy, OMNICEF should be taken at least 2 hours before or after the antacid.

As with other b-lactam antibiotics, probenecid inhibits the renal excretion of cefdinir, resulting in an approximate doubling in A. C. a 54% increase in peak cefdinir plasma levels, and a 50% prolongation in the apparent elimination half-life.

Iron Supplements and Foods Fortified With Iron

Concomitant administration of cefdinir with a therapeutic iron supplement containing 60 mg of elemental iron (as FeSO 4 ) or vitamins supplemented with 10 mg of elemental iron reduced extent of absorption by 80% and 31%, respectively. If iron supplements are required during OMNICEF therapy, OMNICEF should be taken at least 2 hours before or after the supplement.

The effect of foods highly fortified with elemental iron (primarily iron-fortified breakfast cereals) on cefdinir absorption has not been studied.

Concomitantly administered iron-fortified infant formula (2.2 mg elemental iron/6 oz) has no significant effect on cefdinir pharmacokinetics. Therefore, OMNICEF for Oral Suspension can be administered with iron-fortified infant formula.

There have been rare reports of reddish stools in patients who have received cefdinir in Japan. The reddish color is due to the formation of a nonabsorbable complex between cefdinir or its breakdown products and iron in the gastrointestinal tract.

Drug/Laboratory Test Interactions

A false-positive reaction for ketones in the urine may occur with tests using nitroprusside, but not with those using nitroferricyanide. The administration of cefdinir may result in a false-positive reaction for glucose in urine using Clinitest ®. Benedictis solution, or Fehlings solution. It is recommended that glucose tests based on enzymatic glucose oxidase reactions (such as Clinistix ® or Tes-Tape ® ) be used. Cephalosporins are known to occasionally induce a positive direct Coombsi test.

Pentaerythritol

Pentaerythritol

Pentaerythritol is a tetrahydric neopentyl alcohol. It is an odourless white, crystalline, solid compound. It is non hygroscopic, practically non-volatile and stable in air. It is apparently non-toxic and non-violating to the skin. The Pentaerythritol is principally used in the surface coating industries. The medium and long oil length Alkyd resins prepared from Pentaerythritol, dibasic, acids and drying or semidrying oils or fatty acids are widely used in compositions that dry rapidly to form surface coatings with high gloss, excellent water and alkali resistance and good ageing characteristics. Pentaerythritol is an alcohol prepared by treating acetaldehyde with formaldehyde in an aqueous solution of calcium hydroxide; used in synthetic resins and in paints and varnishes.

Pentaerythritol based resin esters are used in a variety of products including paints, varnishes, lacquers, printing inks, floor coverings and adhesives. Pentaerythritol esters of fatty acids are used as plasticizes for PVC resins. Such esters also show promise as lubricants. The poly oxy alkaline ethers of Pentaerythritol have proved useful as surface active agents and as foamed polyurethane resins. Sometimes it is also used in fire retardant surface coatings. Exposed to high temperatures, these coatings froth and swell to produce a solid, non-combustible residue which serves to protect the substrate from fire.

Pentaerythritol tetranitrate (PETN) has characteristics similar to those of cyclonite and is mixed with TNT to form the explosive pentolite. It also forms the core of the explosive primacord fuses used for detonating demolition charges and the booster charges used in blasting.

Alkyd resin paint is esters of polybasic acids and polyhydric alcohols which have been modified by oil or fatty acids; pentaerythritol is used as a polyhydric acid. In particular, alkyd paint made with pentaerythritol has superior adhesion, weather-resistant, color, luster, water resistant and chemical resistant properties in comparison with trihydric alcohols, such as glycerin, and also dry faster than trihydric alcohols.

Almost all synthetic resin paints use oil (drying oil) and to meet with the increasing demand for such oils and offer users better quality than previously available, a number of modified synthetic drying oils can be make by chemically altering synthetic drying oil or natural oil. Pentaerythritol can be used to produce synthetic drying oils of good quality by esterification reaction with linseed oil, tung oil, or other natural drying oils, or by ester exchange and other reactions with fatty acid esters. Such synthetic drying oils dry quickly, and because they have outstanding properties, such as luster, water-resistance, alkali-resistance and flexibility, they are widely used in producing printing inks and processing paper. Ester gum (glycerin rosin ester) has been used as a vector for oil varnish, ink and other substances; the same rosin ester can be obtained by the use of pentaerythritol. Its hardness, high melting point, weather-resistance, high heat-resistance and low evaporation loss and colorization, as well as its anti-oxidizing properties, are the features in using pentaerythritol for many types of varnish enamels. Pentaerythritol and fatty acid esters give plasticity to synthetic resins. That is, pentaerythritol tetra-acetate, when used as a plasticizer for celluloses, acetates and cellulose esters, gives them improved plasticity and impact strength. Also, esters of propionic acid, butyric acid and others are used as plasticizers for cellulose and vinyl chloride. Further, tetraesters of C6-12 acids (caproic acid and peralgonic acid) improve the mechanical, electric and heat aging properties of vinyl chloride, and are of low volatility, making them plasticizers when used for vinyl chloride in those special applications. Pentaerythritol and esters of monobasic long-chain fatty acids (stearic acid, palmitic acid), depending on the degree of esterification, take various physical forms, and are used as emulsifiers in cosmetics, and for abrasives and other purposes. Monobasic fatty acid esters of pentaerythritol have outstanding characteristics as non-ionic surface active agents, and are used in softening processes for synthetic fibers and chemical fibers (acetate, vinylon, nylon) and for smoothing in various processes of synthetic textile production. Because fatty acid esters of pentaerythritol are highly stable, have a relatively high boiling point and are low in volatility, they are used as brake oil as well as lubricating oil and corrosion preventative oil in aircraft engines. If pentaerythritol is chlorinated by means of thonyl chloride and then dechlorinated, 3.3 is obtained. If this is polymerized by BF3 or other compounds, polyether is obtained. Hercules has commercialized this as pentone resin, and because it has outstanding heat-resistance, chemical-resistance and creep-resistance properties it is widely used in mining, refining, plating, papermaking and other processes in similar manner to corrosion-resistant metals, vinylidene chloride and fluoro-resins. In addition, pentaerythritol is used as the polyether material for production of hard polyurethane. Pentaerythritol forms resins by reacting with acrolein, glutaraldehyde and various aldehydes. The resins obtained have particularly good mechanical and electric properties. Pentaerythritol tetranitrate is a stable nitric ester but is sensitive to shock and is used to denotate explosives. Pentaerythritol is used as a medicine to lower blood pressure, as a tackifier in synthetic rubber manufacture, and as a stabilizer in vinyl chloride, among other uses.

Erythroderma (Generalized Exfoliative Dermatitis) Background, Pathophysiology, Epidemiology, Eritrod

Erythroderma (Generalized Exfoliative Dermatitis)

Background

Exfoliative dermatitis (ED) is a definitive term that refers to a scaling erythematous dermatitis involving 90% or more of the cutaneous surface. Exfoliative dermatitis is characterized by erythema and scaling involving the skin's surface and often obscures the primary lesions that are important clues to understanding the evolution of the disease. Clinicians are challenged to find the cause of exfoliative dermatitis by eliciting the history of illness prior to erythema and scaling, by probing with biopsies, and by performing blood studies. See the images below.

The term red man syndrome is reserved for idiopathic exfoliative dermatitis, in which no primary cause can be found, despite serial examinations and tests. Idiopathic exfoliative dermatitis is characterized by marked palmoplantar keratoderma, dermatopathic lymphadenopathy, and a raised level of serum immunoglobulin E (IgE) and is more likely to persist than other types.

The term l'homme rouge refers to exfoliative dermatitis that is secondary to cutaneous T-cell lymphoma. The historic classification of exfoliative dermatitis into Wilson-Brocq (a chronic process associated with exacerbation and remissions), Hebra or pityriasis rubra (relentlessly progressive disease), and Savill (self-limiting) types lacks any clinical significance.

Pathophysiology

An increased skin blood perfusion occurs in exfoliative dermatitis (ED) that results in temperature dysregulation (resulting in heat loss and hypothermia) and possible high-output cardiac failure. The basal metabolic rate rises to compensate for the resultant heat loss. Fluid loss by transpiration is increased in proportion to the basal metabolic rate. The situation is similar to that observed in patients following burns (negative nitrogen balance characterized by edema, hypoalbuminemia, loss of muscle mass).

A marked loss of exfoliated scales occurs that may reach 20-30 g/d. This contributes to the hypoalbuminemia commonly observed in exfoliative dermatitis. Hypoalbuminemia results, in part, from decreased synthesis or increased metabolism of albumin. Edema is a frequent finding, probably resulting from fluid shift into the extracellular spaces. Immune responses may be altered, as evidenced by increased gamma-globulins, increased serum IgE in some cases, eosinophil infiltration, and CD4 + T-cell lymphocytopenia in the absence of HIV infection. Oxidative stress is also associated with drug-induced erythroderma. [1]

Coricidin Cough And Cold Medical Facts From, Corodin

Coricidin Cough and Cold

Generic Name: chlorpheniramine and dextromethorphan (klor feh NEER a meen and DEX troe meh THOR fan) Brand Names: Coricidin HBP Cough & Cold, Dimetapp Long Acting Cough Plus Cold, Robitussin Cough & Cold Long-Acting, Scot-Tussin Sugar Free DM, Triaminic Cough & Runny Nose, Triaminic Cough & Runny Nose Softchews, Triaminic Night Time Cough & Runny Nose, Tricodene Sugar Free

What is Coricidin Cough and Cold (chlorpheniramine and dextromethorphan)?

Chlorpheniramine is an antihistamine. It blocks the effects of the naturally occurring chemical histamine in the body. Chlorpheniramine prevents sneezing; itchy, watery eyes and nose; and other symptoms of allergies and hay fever.

Dextromethorphan is a cough suppressant. It affects the signals in the brain that trigger cough reflex.

Dextromethorphan will not treat a cough that is caused by smoking, asthma, or emphysema.

The combination of chlorpheniramine and dextromethorphan is used to treat sneezing, itching, watery eyes, runny nose, and cough caused by allergies, the common cold, or the flu.

Chlorpheniramine and dextromethorphan may also be used for other purposes not listed in this medication guide.

What is the most important information I should know about Coricidin Cough and Cold (chlorpheniramine and dextromethorphan)?

Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. Do not use a cough or cold medicine if you have used an MAO inhibitor such as isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam), or tranylcypromine (Parnate) within the past 14 days. Serious, life-threatening side effects can occur if you take cough or cold medicine before the MAO inhibitor has cleared from your body. Avoid drinking alcohol while you are taking this medication. It can add to drowsiness caused by an antihistamine. Do not use any other over-the-counter cough, cold, allergy, or sleep medication without first asking your doctor or pharmacist. If you take certain products together you may accidentally take too much of one or more types of medicine. Read the label of any other medicine you are using to see if it contains an antihistamine or cough suppressant. Dextromethorphan will not treat a cough that is caused by smoking, asthma, or emphysema.

What should I discuss with my healthcare provider before taking Coricidin Cough and Cold (chlorpheniramine and dextromethorphan)?

Do not use a cough or cold medicine if you have used an MAO inhibitor such as isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam), or tranylcypromine (Parnate) within the past 14 days. Serious, life-threatening side effects can occur if you take cough or cold medicine before the MAO inhibitor has cleared from your body.

Before taking this medication, tell your doctor if you are allergic to any drugs, or if you have:

an enlarged prostate; or

problems with urination.

If you have any of these conditions, you may need a dose adjustment or special tests to safely take chlorpheniramine and dextromethorphan.

This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment. This medication may pass into breast milk and could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

Artificially-sweetened liquid forms of this medicine may contain phenylalanine. This would be important to know if you have phenylketonuria (PKU). Check the ingredients and warnings on the medication label if you are concerned about phenylalanine.

How should I take Coricidin Cough and Cold (chlorpheniramine and dextromethorphan)?

Use this medication exactly as directed on the label, or as it has been prescribed by your doctor. Do not use the medication in larger amounts, or use it for longer than recommended. Cold medicine is usually taken for only a short time until your symptoms clear up.

Always ask a doctor before giving cough or cold medicine to a child. Death can occur from the misuse of cough or cold medicine in very young children.

Measure the liquid form of this medicine with a special dose-measuring spoon or cup, not a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one.

The chewable tablet should be chewed before you swallow it.

Talk with your doctor if your symptoms do not improve after 7 days of treatment, or if you have a fever with a headache, cough, or skin rash.

This medication can cause you to have unusual results with allergy skin tests. Tell any doctor who treats you that you are taking an antihistamine.

If you need to have any type of surgery, tell the surgeon ahead of time if you have taken a cold medicine within the past few days.

Store this medicine at room temperature, away from heat, light, and moisture.

What happens if I miss a dose?

Since cough or cold medicine is usually taken only as needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and take the medicine at your next regularly scheduled time. Do not take extra medicine to make up the missed dose.

What happens if I overdose?

Seek emergency medical attention if you think you have used too much of this medicine.

Overdose symptoms may include feeling restless or nervous, nausea, vomiting, stomach pain, dizziness, drowsiness, dry mouth, warmth or tingly feeling, or seizure (convulsions).

What should I avoid while taking Coricidin Cough and Cold (chlorpheniramine and dextromethorphan)?

This medication can cause side effects that may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be awake and alert.

Avoid becoming overheated or dehydrated during exercise and in hot weather.

Avoid drinking alcohol while you are taking this medication. Alcohol can add to drowsiness caused by an antihistamine. Tell your doctor if you regularly use other medicines that make you sleepy (such as other cold or allergy medicine, narcotic pain medicine, sleeping pills, muscle relaxers, and medicine for seizures, depression, or anxiety).

Avoid taking diet pills, caffeine pills, or other stimulants (such as ADHD medications) without your doctor's advice. Taking a stimulant together with cough or cold medicine can increase your risk of unpleasant side effects.

Tizanidine - Side Effects, Dosage, Interactions, Tizanin

Tizanidine

Tizanidine is a short-acting muscle relaxer that's marketed under the brand name Zanaflex.

It's used to treat muscle spasms caused by certain conditions such as multiple sclerosis. ALS. or spinal cord injury.

It is also prescribed off-label for certain headache disorders, symptoms of fibromyalgia. and as a sleep aid.

Tizanidine works by blocking nerve impulses that are sent to your brain. If you are suffering from spasms, cramping, and tightness of the muscles, your doctor may prescribe tizanidine.

Tizanidine does not cure these problems, but it may allow other treatment, such as physical therapy, to be more helpful in improving your condition.

Tizanidine Warnings

Tizanidine is a short-acting medication that you should only take for daily activities that require relief from muscle spasms.

Using too much tizanidine can severely damage your liver, causing liver failure.

Tell your doctor if you have any type of previous injury to your liver. Your doctor will likely check blood levels after a month of use if you fall into this category.

It's also important to inform your doctor if you have experienced kidney failure.

Call your doctor immediately if you develop dry mouth, dizziness, a strong desire for sleep, or any sign of weakness while taking tizanidine. Your dosage may have to be reduced or discontinued.

You should not drive, use heavy machinery, or do any activity that requires you to be alert while using tizanidine.

If you are scheduled for surgery, let your doctor know you are taking tizanidine.

Before having surgery, tell your doctor or dentist about all the products you use, including over-the-counter (OTC) drugs.

In older adults, the effects of tizanidine may also last much longer.

Pregnancy and Tizanidine

Tizanidine may be harmful to an unborn baby.

Animal studies have shown an adverse effect, but there are no adequate studies in pregnant women. Tell your doctor if you are pregnant or might become pregnant during treatment.

Do not use this medication without telling your doctor if you are breastfeeding.

Tizanidine 'High' and Recreational Use

Because of tizanidine's sedative effects, numerous online and anecdotal reports have suggested that some people abuse the drug for a narcotic-like "high."

However, tizanidine is not a narcotic.

Take tizandine only as directed by your doctor, and keep this and all other drugs away from children, teenagers, and anyone for whom the drug has not been prescribed.

Tizanidine is available in both capsule and tablet form (2 mg and 4 mg tablets).

The medication in the capsule is absorbed differently by the body than the medication in the tablet, so one product cannot be substituted for the other.

When you have your prescription filled, look at the tablets or capsules in the bottle and make sure that you have received the right product.

Tizanidine also reacts differently depending on whether you take it with food or on an empty stomach, or if you sprinkle the contents of the capsule on food.

The usual starting dose is 2 mg taken by mouth every six to eight hours. A maximum of three doses can be taken in a 24-hour period. Your doctor could increase the strength in 2- to 4-mg steps to help reach the optimum effect and tolerance for your condition.

In order to reduce your risk of side effects, your doctor may direct you to start this medication at a low dose and gradually increase your dose.

Follow your doctor's instructions carefully.

Tizanidine Withdrawal

Tizanidine may cause you to have withdrawal reactions, especially if it has been used regularly for a long time or in high doses.

Withdrawal symptoms, such as anxiety, tremor, increased blood pressure, heart rate, and muscle tenseness, may occur if you suddenly stop using this medication.

To prevent this from happening, your doctor may reduce your dose gradually to discontinue your use of tizanidine.

Tizanidine Overdose

Get immediate help or go to an emergency room if you suspect you have taken too much tizanidine.

Symptoms of an overdose include:

Blurred vision

Chest pain

Confusion
